"Disappointing Experience at Assisted Living Facility"
I first met with someone named [name removed] to tour and start things rolling. She wasn't good at getting back to me when I had questions. Actually, she pretty much ignored all my questions and never replied. It's not surprising she's not there anymore. I almost didn't choose PSV just because of her, but I liked how it looked and the yard in the middle. The other workers were nice, though. No problems with them. My mom passed away on [date removed], and we took all her stuff out of there. We're still waiting for the refund. I left 2 messages for [name removed] but haven't heard back yet. When I went to pick up her stuff, her room was a mess and smelled bad. It needed a good cleaning, and there were a couple of adult diapers lying around. Some of her clothes and her second pair of shoes were missing too.

"Friendly and Caring Staff Make Park Senior Villas Outstanding"
We really like Park Senior Villas because of the people. The folks there are super friendly and caring. That's what we appreciate the most. From our experience, we think other families searching for a place for their loved ones would like it too.

"Understaffed Assisted Living Facility with Beautiful Setting"
The place looks nice, but there aren't enough staff. Some caregivers are great, but others aren't. The boss doesn't help out when there aren't enough workers, which I think is bad. She should pitch in, especially since many patients can't move around much.

"Neglected and Maltreated: A Family's Nightmare in Assisted Living"
Our family had a horrible time here. The person we care about was found with sores all over. They were always hungry and seriously needed water. They didn't get their meds regularly, and when they did, they weren't the right ones.

"Care Lacking at Promising Senior Facility"
Park Senior Villas in Goodyear sounds like a wonderful place. The idea is awesome, but the reality of care is different. I’ve talked to the staff many times and seen the State ombudsman visit in May and June. We found out the manager didn’t have a license, and the place almost lost its own license because it went without one for almost a year. Arizona needs to fix this. Some caregivers are really kind and pay attention to us, but they can’t do everything. I’m not sure if they’re handling medicines right. The acting manager, her family who work as a nurse and in the office, seem to care more about money. Trust issues happen a lot, and the place isn’t very clean or helpful. They might be using meds to control us because they don’t have enough staff. We might be left alone for the same reason. The manager tries to get outside groups and Medicare to pay for what we need by using “preferred partners.” That’s a warning sign.

"Activities lacking, cleanliness inconsistent, COVID protocols lax: mixed experience."
The facility might not be the best choice unless it's close by. At first, they had someone organizing activities, but they left and no one stepped up. I don't know if it's because of COVID or what. I'm not sure if they're cleaning the rooms like they're supposed to. They should help residents shower more often, especially those who need extra care. Some of the staff were nice and helpful, but the management wasn't strict about COVID rules. They didn't make people wear masks, which seemed careless.

"Poor Hygiene and Negligence at Assisted Living Facility"
Only a couple of workers were good at this assisted living place. But lots of stuff was not good, especially when I visited from far away. Once, I saw a worker using his bare hands to get food from the buffet trays to eat, and the residents were allowed to get seconds from the same trays. My sister, who lives nearby, always had a hard time talking to the staff about new health problems. She had to repeat herself lots of times about any health changes. Managers were changing so fast, it was hard to keep up. The food was horrible, and sometimes you couldn't even tell what it was. They were supposed to have a backup dish if a resident didn't like the main meal, but any time my family member asked for something else, they just said "This is your dinner." They didn't do fun activities very often, even though they said they did them daily. One time, my relative called my sister and said, "There's no one here to help me. What should I do?" Another family member got the same type of call and had to call 911. When the emergency guys showed up, there weren't any workers in the memory illness building. All the residents were all alone. We had to wait months to move our family member out because of the virus, but we finally were able to do it.

"Grieving Widow Recounts Husband's Neglect at Park Senior Villas"
On, I took my husband to Park Senior Villas, Goodyear, AZ, a Memory Care Facility, after thorough research. I had many talks with the Marketing Director and a long meeting with their nurse, discussing my husband's prescriptions, daily needs, health problems, and past medical history. The nurse carried out some tests and wrote notes. They even received medical files from my husband's three current doctors: an Internist, Neurologist, and Urologist. The Marketing Director assured me that every worker had taken an Alzheimer's course to understand their residents' needs better. I filled in their necessary forms, listing my husband's daily needs like catheter care, blood pressure checks, meal notifications, toileting aid, help with bathing and moving around (I supplied a walker and shower chair), and noting that he had fallen a few times due to foot shuffling, confusion, and other needs. I also coordinated with Sage Hospice, Scottsdale, AZ to go through all this information and secure their services. They told me a registered nurse would initially visit my husband twice a week under their doctors' guidance. With these assurances from Park Senior Villas and Sage Hospice guaranteeing I'd be notified immediately of any changes to my husband's condition, I admitted him on feeling satisfied with the care level expected. Due to Covid-19 regulations, he was isolated for 2 days with regular caregiver checks. Visits were limited, and relatives were urged not to visit for two weeks to help the new resident settle in. I was told I could call anytime to check on him. On , I made plans to visit him, but before I could, I got a call from the hospice nurse about his low appetite and a cough. Shortly after, the facility called, asking if I had a Do Not Resuscitate order because my husband's blood pressure had dropped significantly. I rushed him to the hospital, where I found out he had pneumonia and severe dehydration. His kidneys were failing, and dialysis wasn't an option. After four days of treatment, though tests showed minor improvement, his condition wasn't improving overall. He passed away 28 hours after being brought home on When I saw my husband at the hospital, I noticed he had lost weight, looked pale, wore a dirty shirt (the same one he had on when admitted), and hadn't been shaven for days. Compared to when he first arrived at the facility, he was barely recognizable. My husband's passing after 52 years together is devastating, worsened by Park Senior Villas and Sage Hospice's alleged neglect.

"Unsatisfactory Staffing and Harsh Environment at Assisted Living Facility"
This place at first seemed perfect for my elderly father who needs a little extra help with daily tasks. I visited twice, both during the day and in the evening. Everything seemed okay during the daytime - the staff was visible and performing their duties well. In contrast, the evening staff seemed absent, verging on neglectful. One visit, I didn't observe any caregivers. The second time, I practically had to awaken the on-duty caregiver. At this point, I was hesitant about the idea of placing my father there. Despite my concerns, I paid a third visit, during which I conversed with a caregiver. I asked why there were hardly any staff around. The response was disheartening: apparently, the employees don't stick around due to ruthless remarks from residents specifically targeted at caregivers of color. Furthermore, they feel unsupported in their roles. Naturally, if the staff are treated poorly, it's unlikely they'll stay around, or that they'll provide quality care for the residents. Ultimately, I'm relieved that I didn't choose this facility for my father.

"Facility changed ownership without notice, but clean with good food."
When my mom first moved there, it was a different company. My main problem started when the ownership changed. They didn't inform us; we just realized it because many staff members left. The one staff member we knew said the company changed hands and she was upset about it. Since then, my mom passed away. The place looks nice, the food was tasty, and it was always clean.

"Neglectful Care and Broken Promises at Park Senior Villas"
Park Senior Villas might look good at first glance, but once you take a closer look you can see how messed up it really is. During the four months my mom was there, she only got to shower three times. The workers who were supposed to give her medication often forgot, and I don't know how many times exactly they messed up. For a lot of nights, the residents had no one looking after them. Seriously, there were two whole days where no caretakers were in the home at all. We had multiple sit downs where we were promised things would get better. We even talked to the person who owns the place but nothing changed. In fact, the situation got even worse. There were several times when the police and fire department had to come because there were no workers on site and a resident's family couldn't get into the home. One resident had fallen and laid on the floor for over two whole hours with no one there to help. We were paying about $5000 every month just for a spot to live and all we asked for was basic human care. My advice? Stay far away from this place. I wouldn't suggest anyone put a family member here. The lacking care we got at Park Senior Villas is beyond belief.

Went here to find a spot for my mother. This place is not what I was hoping for at all. Definitely, visit it first before you make any decisions.

"Salesperson Dishonest About Insurance Acceptance"
The salesperson was less than honest, similar to a shady used car dealer. She wasn't truthful about the types of insurance they accept. Now she claims they might start accepting our insurance in a year and a half.
