"Awful Care and Miscommunication at Heritage Park Assisted Living"
My father-in-law got sent to Heritage Park (HP) after a partial hip replacement due to a fall at San Antonio Hospital in Upland. Soon afterward, visitors couldn't visit because of Covid-19. Two weeks in, my sister-in-law finally met with his supposed care team, including a doctor, only to find out that they hadn't even begun his physical therapy yet! His so-called care team might as well have not existed. They didn't even give him pain medication until we asked about it. They started him on Oxycontin after that, and he just seemed drugged up for a month! He couldn't even keep a phone conversation going without falling asleep mid-sentence. The staff seriously needs some improvement - nobody is ever around to answer questions. They only let him go once his Medicare ran out, without letting us know he could have been transferred to another place. He still couldn't stand on his own, use the restroom, do basic tasks that he normally could, but they sent him home. They only gave him half a tank of oxygen for his COPD. Please don't pick this place if you have alternatives, the patient treatment is seriously terrible. By the way, he can now have a full conversation after they took him off the Oxycontin.

"Review: Poor accommodations and care, inadequate staff, and subpar food."
I definitely wouldn't suggest this place for anyone. They really need to do better with the care they provide. The AC doesn't go under 85, which is so awful. Plus, there's part of the carpet that's ripped where people have to walk, and it's totally a tripping hazard. The food isn't great either; don't be fooled by the 'turkey dinner' it's just a slice of lunch meat with gravy on top. Trying to talk to the night staff? Forget about it. They're pretty much nonexistent to residents and families. When you do manage to talk to the nurses, they seem friendly and promise to sort things out, but don't hold your breath; I've been waiting for things to change for a whole 4 weeks now. I want to give credit where it’s due; a couple of the nurses do seem to care. But they’re overwhelmed because there aren't enough staff, and unfortunately, they don't have any control over the air or the food. Overall, the level of care provided here is just not good enough.

"Negligent Facility: HPNC Mistreatments Lead to Family Rescue"
HPNC Misplaced My Mom's Phone It took HPNC 7.5 hours to insert an IV in my Mom, even after receiving doctor’s orders. This lengthy delay demanded some family members to intervene late in the night to ensure the IV was started. My mother's water was kept just out of her reach and her breathing treatment device was left on long after her treatment was over. She wasn't provided with substitute meals when she couldn't eat the meals on their menu, leaving her hungry. A nurse wrongly stated the timing of a blood test, saying it was done when it actually wasn’t. Throughout their care, they didn't make an effort to get her out of bed. Her iliostomy bag leaked for 8 long hours while my Mom repeatedly asked it to be changed and cleaned up. This neglect led her to lie for over 8 hours in her own urine and feces, resulting in a fungal infection which required treatment at a local hospital. During her stay at the HPNC, they administered Robitussin DM-Dexthomorphan and Guafenisin, which possibly caused hallucinations due to their interactions with her other medications. We had to "RESUCE" our mother from the inadequate care of the HPNC. Not only did they fail to provide quality care, but they also refused to share any documentation related to her care at the time of her release, which law obligates.

"Family Dissatisfied with Assisted Living Facility's Care for Mother"
HPNC misplaced my mom's phone. It took HPNC a whopping 7.5 hours, which included some firm insistence from our family, to finally get around to put an IV in my mom after the doctor had ordered it. They put her water out of arm's reach. They forgetfully left the breathing treatment device on her long after the treatment was finished. They failed to provide different meal options for food she wasn’t able to eat, leaving her with an empty stomach. The nurse shared incorrect details about when a blood test was done, claiming it was completed when it actually wasn't. There seemed to be no efforts to get her out of bed. Her iliostomy bag was leaking for 8 hours, resulting in her laying in her own waste for way too long. This caused a nasty infection that required treatment at the local hospital. While in HPNC's care, they gave her Robitussin DM-Dexthomorphan and Guafenisin which, when mixed with her other medicines, might have triggered some scary hallucinations. Our family had to quite literally 'rescue' our mom from HPNC. They even resisted providing us with her care documents upon discharge, even though it is lawfully necessary.

"Mixed Experience at Assisted Living Facility"
My mom has been there about three times, recovering from different things. The physical therapy department was really great, I thought. Some of the nurses and caregivers were really, really good. The place itself wasn't that great. The food was good, but they had a problem with the air conditioning. It was very hot when I went to eat with her, but the food was good, and like I said, the physical therapy was great. There were a few nurses that I wasn't sure about, but overall, it was good. Outside they had a nice area, but there were no umbrellas so you were sitting directly in the sun.

"Great Physical Therapy"
My mom has been there about three times, recovering from different things. The physical therapy department was fantastic, in my opinion. Some of the nurses and caregivers were really good. However, the facility itself had some issues. The food was good, but they struggled with air conditioning. It was sweltering when I went to eat with her, but the food remained tasty, and as I mentioned, the physical therapy was excellent. There were a few nurses I wasn’t sure about, but overall, it was a positive experience. Outside, they had a pleasant area, but unfortunately, there were no umbrellas, so you ended up sitting directly in the sun.

"Great Care and Homey Feel at Heritage Gardens"
My mom has been at Heritage Gardens for about 3 months. They take great care of her, the staff is fantastic, and she loves the food. I visited many places before finding this one. I wanted a place that felt like home, not too big or expensive, with activities to keep her busy. Heritage Gardens has everything I was looking for and more. I’ve talked to other families with loved ones here, and they’re all happy with the care their family members get. I’m really glad I chose Heritage Gardens.
