The staff here are pretty good. Some days are better than others. The cleaning is good. On weekends, they could use more help. They keep me informed about any issues with my mom. The meals are okay. I would suggest visiting first and being friendly with the staff. That's good advice for any place like this.

"Don't Trust this Facility: Poor Communication and Unresolved Issues"
Just the Facts: We tried for 6 years to talk to the people running this place, but they barely paid us any attention. Even when they did, they never did what they said they would. The worst part was when our relative died there. We are the closest family he had but they didn't tell us about his death. We found out about it by calling the morgue. A person they called "POA" took everything from our relative's room, including his TV, medicine, and even a gun. We finally got a message from the manager asking us to clean out his room, but by then, most of the stuff was gone. The things that mattered to us, like pictures and Bibles, were all that was left. Please take your loved ones far away from this place. It's not safe and the people running it are shady.

There's been trouble with bugs. He lives on his own and doesn't need help. He makes his own breakfast but isn't happy with some meals. He feels it's pricey, especially since the rent increased after just moving in March.

"Mixed Experience at Fountain at Mills Assisted Living Facility"
The Fountain at Mills was alright overall. Sometimes, the cleaners forget to tidy up, but they're only supposed to clean on Tuesdays. Some of the staff are friendly, but others are not, and a few are lazy, especially on weekends. The facility should have a manager on duty during weekends, but they're often absent, and it's hard to reach management. The quality of care is okay for those who can speak up for themselves, but it's not suitable for people like my brother who's in a wheelchair. The meals are great, but activities are only okay, and sometimes they forget to remind residents if they're not downstairs on time. The costs are high, especially for veterans, as they increase yearly. I'm waiting for a space at a VA facility for my brother.

This assisted living facility is affordable and has friendly staff. Sometimes, there aren't enough staff on weekends, which might happen in other places too. "Needs Improvement, Aunt Content"
At first, we had some trouble with care and still have issues with paperwork for insurance. The outside of the place really needs work, like fixing things, cleaning, and taking care of the gutters (there’s even a little tree growing in one!). There’s a lot of mold and green stuff that isn’t good for anyone there. The ponds look dirty too and the fountains are broken. They should fix it up because it’s even in the name of the place. It would be great for health and make everyone feel better to have a clean outside area. Inside is old-fashioned and should have new floors that are easier to clean and don’t hold onto germs and smells. But overall, my aunt likes it there and she’s doing okay.

My loved one lives at Fountains Residence at Mill Cove. Everything here has been really good. The people are friendly, the place is clean, and the staff are nice. I haven't tried the food or activities, so I can't say how those are. But overall, I would tell my family and friends to consider this place.

"Inadequate care led to medication errors and unfair eviction. Avoid."
They messed up the medicine doses and timing. When I asked, they said they only had one nurse for 40 people. They were busy with other patients. When my loved one got upset because of it, they made him leave the same day, but still made us pay for the whole month. He had only been there for one week. I wouldn't tell others to go to Fountains Residence at Mill Cove.

"Stinky Spot with Untrained Nurses"
This place isn't great at all. One of my family members stayed temporarily to get stable. Maybe it's a bit better when someone is in the Assisted Living section. But when they get moved to the Nursing Care or Hospice areas, things get way worse. The whole place has a stinky pee smell and the nurses really don't know what they're doing. Plus, they don't show caring or even bother to communicate properly. I would tell everyone to stay away from this place.

"Focused on Profit; Lacking in Care and Cleanliness"
My dad lived here for a few weeks before I decided to move him elsewhere. This place is quite big, but seems largely focused on making money. They were originally supposed to evaluate him before his arrival, but to my knowledge, that didn't happen. The care my dad required seemed to be beyond their means unless we paid extra. During his stay here, I also found myself leaving work frequently to assist him in moving to the dining area and giving his medicines. In the dining room, it often took up to 15 minutes before they would serve him. The place looks wonderful, but it has an odd odor similar to a gas station restroom that isn't cleaned properly. Once I let the staff know I was going to move him, their behavior shifted and they were suddenly eager to assist my dad.

"Appreciated Care Giver: Mom-in-Law Loves It"
We're pretty satisfied with how my mother-in-law is treated. The workers are always kind. She really enjoys the food that's offered. She's also a big fan of the many activities they've got available. Plus, if she feels like cooking in her room, she has the freedom to do that. The place is conveniently located close to where we live. We'd suggest this place to other families seeking quality care for their loved ones.

"Wonderful Experience So Far, Excited for the Move!"
My mother hasn’t moved into this community yet, but our experiences have been wonderful so far. This place looks really nice, and my mom is super excited to move in. They even invited us to a dinner before the move, and the food was delicious. Everyone here is very kind and friendly. A Place for Mom was amazing in helping us, especially my advisor Laura. She was outstanding and made this process so easy!

"Distressing Lack of Care at Brookdale Rehabilitation!"
My Dad was moved here for recovery. As a doctor, I found quite a few issues with their way of looking after my Dad: 1. He's a diabetic with kidney disease in its final stages. It looks like they only know how to work with one type of diet. They put him on a kidney-friendly food routine, totally disregarding that he is a diabetic! They fed him more carbs than he's had in a while - pancakes, brownies, cookies, pasta, mashed potatoes, spaghetti. His blood sugar reached alarming levels during his stay at Brookdale. 2. They messed up his medicine! They reduced the amounts of his statin and blood pressure medication! 3. They didn't bathe him or change his bedding for almost a week! Even though there's supposed to be a "scheduled time for bathing", my Dad didn't get bathed for six whole days! 4. Except for the social worker and Admissions Coordinator, the staff didn't care about my worries! I had to wait four days for a call back after leaving multiple voicemails every day about my concerns. 5. It took nearly three days before his first physical therapy evaluation...and he was moved there for physical therapy in the first place! They don't have enough staff and for a couple of days, there was only one physiotherapist onsite for everybody's needs. 6. Their nutrition room was downright gross, with cups that looked like they were from days ago (dirty) being kept for future usage! 7. They're breaking Joint Commission standards! I saw nurses having meals at the nurses station. 8. Most of the times I tried to reach them, nobody picked up the phone!

"Great place with important church services for everyone!"
After my husband passed away, I had a stroke and couldn’t live in my home with stairs anymore. My sons and I looked at different senior living communities around Jacksonville, and we all agreed that Brookdale Atrium Way was the best choice for me. I’ve been here for over six years, and I’ve made some wonderful friends. I really enjoy the activities they offer, and I especially appreciate the church services because attending church is very important to me.

I moved to Brookdale Atrium Way a few years ago because I really liked the people here. I checked out other retirement communities, but none were as friendly as the Atrium. All my friends live here, and I enjoy all the activities and events. Living here with my friends is wonderful!

"A Community That Feels Like Home"
My wife and I moved from St. Augustine to Jacksonville to live in a retirement community. At first, Brookdale Atrium Way wasn’t our top choice. However, out of all the places we visited, it was the only one that gave us a warm feeling we can’t quite explain—we just felt at home. We’ve made some great friends here. When my wife, passed away last year, many residents and staff, even those who didn’t know us well, came by to offer their condolences. That really helped me cope. It was comforting to know I had a support group. Even though my daughter suggested I move somewhere else, I decided to stay because this truly feels like home to me. I started volunteering a few months ago, helping with bingo and delivering mail and packages, and I enjoy every minute of it.

"High Prices and Poor Communication at Brookdale Atrium"
My mom has been at this place for a year and she's alright there. The place is okay, but the real issue is the cost! If you're taking care of your finances, Brookdale isn't the place to be. Surprisingly, they seem to think old folks get a $150 raise every year. We weren't warned about this when we signed her up, and now we're in a pickle. There's no explanation for these yearly price hikes--no enhancements, no changes--just a plain rise in cost. I've been trying to talk to the higher-ups at the Atrium, but none of them respond to my messages. I've left separate messages for the daily care staff about needed assistance, but still, no one has called me back. If I'd known all this earlier, I'd have second guessed bringing my mom here. I'm not happy with Brookdale Atrium, it seems like they're just about making money, with residents just seen as another income source.

"Janet went above and beyond!"
We had a great time dining with family last Sunday in a private dining room. There were 10 of us from four generations. Our server, Janet from Puerto Rico, was wonderful. She was friendly with everyone, calling our 7-year-old grandson "handsome" and our 14-year-old granddaughter "gorgeous." She was sweet with our 5-month-old baby and our 94-year-old mother too. Some of our group arrived late, but Janet didn’t rush us. She made sure all our food was fresh and hot. Janet is a real asset to the staff, and we felt lucky to have her. All the staff is kind and helpful, but Janet really went above and beyond! My grandmother loves living at Atrium, and after this experience, I wouldn’t want her anywhere else!

I visited this assisted living community, and it’s one of my top choices for my loved one. The place is bright and clean, and everyone seems really happy. When we arrived, they greeted us by name, and almost every resident took a moment to share how much they love living there. The sales director was great to talk to and answered all of our questions. I truly believe my loved one would be happy here, and I can’t wait to move him in!

"Unsatisfactory Care and Management at Assisted Living Facility"
I strongly advise against choosing this living care facility! If you deeply value your loved one, seek a higher-quality place! This facility has a high turnover rate of both nurse techs and nurses. Only 2 nurses demonstrate adequate professionalism, and competence, and adhere to proper care standards. The administration lacks the professionalism, education, and experience required to efficiently manage a nursing home. The majority of the nurse techs, admittedly, are pleasant. However, you always find yourself double-checking if meds were timely given. Patients are not regularly changed nor properly bathed. Unfortunately, there are way too many negative incidents at this facility! I wouldn't even send my beloved pet here! As a healthcare professional, I grade this facility an F!

"Safe, friendly living with great activities and transportation options"
We haven't needed any care services since it's independent living. My mother lives there, and I really like how safe it feels. It’s a very friendly place. The staff talk to the residents and always call them by name. They also provide transportation for shopping and medical appointments. There are exercise classes and entertainment on Friday nights. Plus, they have a country store for toiletries and other items. Each unit is a studio with a small kitchenette and a porch, which my mother really enjoys.

"Would choose this facility again despite some concerns"
Mom has been at the assisted living facility for 7 days. During this time, she fell in her room and had to go to the hospital. The staff handled it well, but they only found out because she was screaming, not because of her emergency pendant. Someone in the hall heard her after about 20 minutes and helped her. I’m still waiting for feedback from the staff about this. The place is clean, the staff is friendly, and there are lots of activities, though Mom hasn’t joined any yet. The cost is high at $5,500 a month, with $3,500 for rent and the rest for support fees.

"Unfulfilled Promises and Depressing Care At Assisted Living Facility"
Despite a couple of bright spots, we were greatly disappointed with my dad's living conditions at this facility. It really didn't live up to the promises that had been made to us about his care. The majority of the employees were hardly interested in answering our questions or meeting his needs. The atmosphere of the place was gloomy and not at all welcoming. There was a constant whirlwind of changing staff who barely knew their duties. Their record keeping was a total disaster. Though I repeatedly voiced my grievances to the director, things didn't get any better. We couldn't get my dad out of there soon enough. There were just two good things - a sincere and hardworking speech therapist and a trustworthy nurse. Even some staff members advised us to seek another place for my Dad.

My family checked out this place while we were trying to find care options for my relative. It looked nice, and everyone who worked there was really nice. But, it's pretty big. Our relative has trouble moving around, so navigating a place this size would've been tough. Plus, it seemed really crowded and busy, which we weren't too keen on.

"Spacious Apartments in Welcoming Assisted Living Community"
I really liked this place. It's really good and clean. My dad likes the care here. They have a lot of gatherings, like card games and movie nights. They also introduce people to each other. They had a nice Thanksgiving celebration for families. Each apartment has a small kitchen with a microwave and refrigerator because they provide three meals a day, and the food is very good! The apartments are much bigger. The building is a bit old, but that's why the apartments are so big. It's unique in that way.

"Atrium: Not for the Wanderers!"
My grandma was able to leave there too much because they don’t lock the doors. She kept ordering taxis and hopping on the bus, so they told me they couldn’t keep her. I was bummed out, as this place was so much more handy and Grandma seemed happier at "Atrium". I'd give them three stars. It was definitely better for her freedom, but it didn't work for her lifestyle. They never told me that if she started to wander off, they'd need to move her elsewhere. They gave me a call one Friday to say she had wandered off and had to leave that same day. I wasn't happy about how they dealt with it. I suggested making some changes to her room, but they wouldn't help me with any of my suggestions.
