"Mom’s Favorite Assisted Living"

Mom loved this facility the most! Nurses and staff were wonderful, caring, and respectful. Building needs upgrades, but Mom’s happiness matters most. Amazing friends, progress after stroke, and heartfelt goodbyes.

"Great care and fun activities at this assisted living facility"

This is a good place for my loved one. The staff really take care of him and work well with me, too. He enjoys different kinds of food and prefers country cooking, so sometimes he has trouble with the meals. But overall, they do a good job. There are activities like coloring, puzzles, workbooks, and more to keep everyone entertained and happy. They also have cooking classes, which he likes. Plus, they arrange outings for the residents to get out and do things in the community.

"Attentive staff, thorough care, and good value at assisted living facility."

The staff at this assisted living place are really caring. No one gets ignored. If someone seems shy or spends time alone, the staff make sure to include them and help them join in. They clean our rooms really well once a week. Every day, they sweep, mop, and take out the trash. They check if we need anything during shift changes and introduce the next staff member. They genuinely want to help us. We wear monitors around our necks for help, and they respond quickly, usually within 10 minutes for non-emergencies. We recently got a new activities director, and she's been great. Overall, it's a great value here.

"Care and comfort"

This community is great for care and has wonderful staff. The menu has improved, and we appreciate how clean it is. The environment is nice, and we would recommend this place for care.

We are happy with the care at this community. The staff is caring and pays attention to the residents. It's always clean and a nice place to be. The menu looks good, and they offer activities for those who can join. We would recommend this community to care for a loved one.

"Quick Tour Reveals Cold Institution, Despite Nice Appearance"

This place was okay, but the folks there didn't seem too friendly. They kept going on about how yummy their grilled cheese and hotdogs are. Sure, it looked like a decent spot. But, it felt more like a cold institution, and I don't think my family member would feel at home here.

"Decent So Far, But Still Improving"

My mom is still getting used to this community. Generally, I'm okay with her living here, but I do worry a bit. Moving her in wasn't the easiest, but they've been helpful since then. The place is clean and well-maintained. They have fun activities. There are still some things that need improving, but overall, it's getting better.

"Friendly Community, But Not Enough Care for Severe Needs"

This is a nice place, but my mother-in-law needed more help than they could give. They tried their best, but she was getting worse. The staff is friendly and very helpful. They even worked with us on the price when she needed more care. Overall, it was a good experience and a kind community. I would recommend it.

"This Is the Place for Mom!"

They've been great during the tours and early process. My mom couldn't move in as planned due to rehab, and they've been really understanding. We picked them because her friends and church group live there, and we wanted her close to her social circle. They offer lots of activities and have a wonderful atmosphere. When we toured, my family all looked at each other and thought, 'This is the place for Mom!'

We took a tour of this community, and it was great, just like all the other Brookdales. I’ve always liked how Brookdale runs their communities and the kind staff they have. The community is clean and tidy.

"Great staff, happy loved one, highly recommend!"

The community helped us with financing. The staff is great and understands our family. My loved one enjoys the food and has different choices. They have many activities to keep the residents busy. The community is clean and well cared for. I would recommend this place.

"Dissatisfaction Over Brookdale Orange Park's Dementia Care Services"

Knowing what I know now, I wouldn't recommend Brookdale Orange Park Memory Care for anyone's loved one, especially if they have dementia. It's a memory care unit, but the staff isn't trained to handle dementia patients. They promised a lot of activities, but most of the time my relative just watched TV in a small room. There's too much sitting and not enough stimulation. The Memory Care Director should be managing things, not just sitting in the front office. We've filed complaints, but nothing changes, it's just an endless talk. The management here is really poor and it's extremely hard to even reach somebody. No one returns calls. Please, do your research before choosing this place.

"Avoid this Facility: Terrible Staff and Negligent Care for Residents"

The care team at this facility leaves a lot to be desired, with their lack of attention and disregard for the individuals living here. My family member had a fall, and was essentially ignored; placed back in bed without any medical examination, dressed as they had been. As it turned out, they had a broken hip and had to endure hours in pain before we arrived. We found their room was locked, and when we sought help, we were informed the maintenance person - who had the key - was unavailable for the day. Even after involving the fire marshal, the issue wasn't rectified. Meanwhile, when we question the nurses about our loved one's condition, we're answered with silence. This place purportedly provides a bell to each resident, on a lanyard, so they can call for needful assistance. Yet, when my family member used theirs, they received no attention for over half an hour. Neither nurses nor other team members were anywhere to be found. Strikingly, even when three nurses passed by, no help was offered. After this, I can't justify recommending this facility to anyone.

"Lack of Care Results in Hospitalized Senior, Facility Abandoned"

They don't make routine checks on the seniors like they're supposed to. If a room door is locked, the staff members have to hunt down the keys. One senior citizen fell and ended up in the hospital with a broken bone and pneumonia. That senior has since left the facility.

"Great Care and Support from the Staff!"

This place is really welcoming for families. The management team is always ready to help with any problems that come up and will return your calls if you need assistance. I would definitely recommend this facility to anyone!

"I’m very happy with the friendly staff and care."

I love it here! The staff is really friendly. A Place For Mom has been amazing. My mom is getting used to her daily routine. I think she enjoys having three meals a day and spending time with other people her age. They call me right away if something happens. I’m so happy with their care!

"Serious Concerns Raised About Assisted Living Facility"

Unsatisfied with Care at Memory Unit, Urgent Improvements Needed." The memory unit at this assisted living facility needs major improvement. There's a terrible smell that's hard to ignore. And finding a staff member or director to help can sometimes be a challenge. When I asked about the director's whereabouts, staff casually mentioned she was probably hanging out with her friend, who happens to be the Executive Director. The medical team's competence and professionalism are subpar, and their attitudes are often unfriendly. The facility needs more compassionate staff on the ground. They do offer some activities, but there should be more options for residents. The Director of Memory Care doesn't seem to have a clue about her job. Her habit of addressing residents with pet names like 'baby' or 'honey', instead of their actual names, rubs me the wrong way. I would hesitate before recommending the Memory Care Unit, particularly for residents who tend to wander. My loved one will be moving to a different facility soon. The current one permits residents to roam into other rooms unsupervised, and falls frequently occur. This place needs a word of caution. It's just an okay community, lack warmth you'd expect in a place caring for the elderly. Some of the front office management team's conduct is awful, dressing more appropriately for a night out than a professional environment. The change in management hasn't made a positive impact.

"Caring Staff and Great Activities at Assisted Living Facility!"

The staff here is amazing, and there are lots of daily activities to keep everyone engaged. They have medical care and help with prescriptions right on site. The place is new and very clean. The management team is also really helpful, making sure we got all the paperwork we needed. We could even decorate our mom's room to make it feel more like home. The staff makes sure she takes her medicine and helps her get to the dining area and back to her room. Their dementia services are top-notch, and everyone is very caring. You can visit any time to see how things are going. It's a wonderful place, and we’re grateful to A Place for Mom for the helpful info!

"Good Facility and Staff, But Pricey"

I thought this place was pretty good and She was awesome. The only issue I had was it's too expensive. They had a special offer that made the deposit and rent cheaper, but I'm not sure if that's happening anymore.

This place turned out to be really good for the price we were looking for. The grounds were great, and the food service is fresh daily, prepared by the on-site chef.

"Questionable Cleanliness and Size for Price in Elderly Home"

This facility seemed aged and only somewhat tidy. They seemed surprised by our visit, yet they managed to show us around. The rooms are quite cramped, especially considering the cost and the fact that you'd be sharing with someone else. Honestly, I'm not convinced about it. Honestly, would I choose to live here? Probably not.

"Mixed Feelings"

I wish they'd talk more with us and introduce other staff, not just managers. They don't tell us about the nursing care there. I'm not happy about how they handle her insulin; they use an outside nurse instead of doing it themselves. Also, I asked to pick up her meds, but they insist on using their own pharmacy, Omnicare, which costs a lot. But overall, my mom's okay there! I just wish she didn't have to be in memory care yet. She's not quite ready, but she wandered off twice, and the police had to bring her back. In assisted living, they can go outside with a buddy, but in memory care, they're stuck in a tiny area. I want them to let residents walk around more outside.

"Location convenience outweighs cultural food differences in assisted living."

My mom doesn't do much other than eating at the assisted living facility. The food's okay for me, but she's not a fan because she's not used to Southern cuisine. We picked this place mainly because it's close, and we didn't want to travel far. I had to explain to my mom that it's not a nursing home, but she comes from a different time, so it's a bit challenging.

"Mixed Experience at Assisted Living Respite Facility"

My dad stayed there for a month for some rest, but it was just okay. Now he's back home. Mom needed a break, but it was pricey. Dad mostly keeps to himself, so he didn't really join in activities. They had to bring food to his room a lot. They did try to get him involved. A couple of caregivers even came to invite him for lunch. I was only there for an hour and a half, but everyone seemed happy. The place looked nice and clean. It took them a bit to get everything ready for him, though. They had a bed, but no TV. I'm not sure if that's normal.
