"Decreasing Care and Unprescribed Medication Concerns"
Our family member has been living here for a good while. Things were okay when she was more independent, but as she got sicker and needed more help, the care provided by the facility faded. We're very upset with how the workers have been handling things, plus the lack of communication is worrying. It seems like they aren't prepared to take care of people who need lots of aid. Moreover, we're pretty certain our relative is getting medication that a doctor didn't prescribe. We even have some proof to back this up.

"Needs Improvement: Memory Care Facility Review"
My mom stays in the basic Memory Care unit, and things aren’t going well. The residents seem inactive and don’t really talk to each other. When I press the help button, it takes a long time for someone to come, like 20 minutes. It feels like they don’t have enough people working to look after everyone. I’ve tried to talk to the person in charge by leaving messages, but I haven’t heard back. The food isn’t great either; it’s pretty tasteless and too thin. They offer activities, but it looks like my daughter and I are the only ones who bring her, or else she just stays in her room. Also, the place feels old-fashioned. When I need to go out, there’s no easy way to open the doors, and the workers don’t offer to help.

"Would've Expected Far Better From This Facility"
My mom's in Level 1 Memory Care and things aren't great. Folks there hardly engage with others, they're pretty unresponsive. When she needs help, the response is slow - pressing the call button once had me waiting 20 minutes. Seems like they lack adequate staff for the residents. I've tried contacting the director, but I never got an answer. Then there're the meals - tasteless and watery. Yes, there are activities, but it seems that only my daughter and I are assisting my mom to them, otherwise she's just stuck in her room. The building is dated; there are no automatic door buttons, and the staff won't lend a hand.

"Feels like home!"
Everything is wonderful here. The staff is amazing and really cares about the residents. The food isn't always the best, but they do offer healthy choices. The community is beautiful and they have lots of activities. My loved one loves the exercise classes and likes that the rooms are big and bright.

"Disappointed with Care Level for Disabled Residents at Great Oaks"
After moving my mom into Great Oaks, they shifted her to a new section within a month because of the staff-patient balance and how long it took to fulfill her needs. Even with some mental confusions, she was social, made friends, and could express her wants. She needed a special sink because of her powered wheelchair, which her first room didn't have. To better meet her accessibility needs and their own staff allocation, they moved her to the memory care area. Sadly, this meant she lost her powered wheelchair which made her really upset, and now she was surrounded by less socially active residents. I've had trouble with response on issues I've raised and unreturned phone calls. Although, the rooms are clean and nice and the care staff haven't given me any issues. I believe the facility could organize care based more on staff-to-patient ratio, particularly for residents needing more physical care. It feels like my mom was moved to the memory section prematurely. So, I'm let down based on what I initially expected for her care at Great Oaks. I picked this place because it was closer and cheaper than another place I checked out.

"A caring, safe home for Mom!"
My mother has lived at Great Oaks for over seven years. She moved from assisted living to memory care in the Gardens as her needs grew. The staff provides excellent care and is very kind. She often says, "They take care of me." They help her with personal care, both physically and emotionally, and they always keep the family informed about any concerns. Even though she misses her family, she feels happy and sees Great Oaks as her home for as long as she needs it.

Overall, our experience with Great Oaks has been a good one! The staff are all very friendly and professional, and they make sure that your loved one gets the best care. They're really good at noticing if something isn't working out or if care needs aren't being met. We really appreciate how they worked with us to fix everything! One of the big reasons we chose Great Oaks was because they have several secure outdoor areas for their memory care patients. Mom loves to be outside, but she needs to be in a safe place.

"Mom Passed After Unauthorized Medication Changes at Facility"
Before my 90-year-old mom moved to Great Oaks, she was healthy and enjoyed eating, despite her dementia. We gave her ativan for the plane ride and told the staff not to give her more because it lessens her quality of life. An employee who isn't a doctor set up a doctor's appointment and got an evening dose of ativan prescribed without telling or asking me. Then, the same employee scheduled another appointment with a different doctor I didn't know and got her usual medication replaced with a morning dose of ativan, all without my approval. They told me she wasn't eating much, which makes sense because ativan not only limits her, but also suppresses her appetite. A week after the second doctor visit, I was told her medication wasn't balanced and she needed hospital care. The doctor who said this was the same one who changed her medication! I tried to put her back on her usual medication and stop the ativan, but it was too late. After only 32 days at Great Oaks, she was sent to the hospital for malnourishment and dehydration. Less than two months later, she had passed away.

"Great Oaks is a wonderful place!"
My mother often complains, but after we moved her to Great Oaks in Monroe, she was so happy! She praises her room, the staff, the care, and the food. My family is very satisfied. They keep her active with many activities. When the weather is nice, there's a gardening area where residents can plant flowers. This is great for my mother because gardening was one of her favorite hobbies.

"Paid Too Much For Too Little Care"
The cost was pretty steep considering the little amount of care we received. We initially sought a one-bedroom apartment and ended up paying extra just to secure it for him. The activity offerings were commendable, however, he didn't join often because his memory care caused him to spend most of his time in his room or on the patio smoking. He struggles with recognizing his need for medication, so he needed encouragement to take them. Fortunately, the nurses were helpful on that front; but he ended up getting kicked out as we were making adjustments to his medications.

"Great place, nice lunch, and many activities offered!"
I toured the facility with the director and had lunch. The place is nice and a little bigger than we wanted, but the price is better at $2,600. They have a pharmacy on site and many activities.
