"Assisted Living Disappoints with Staffing Turnover and Lack of Activities"
We gave two stars because some things weren't as promised during our first visit. Many staff come and go quickly, and they don't try hard to plan things for the residents to do.

"Mixed Experiences at Charter Senior Living; Staff Praised, Communication Lacking"
The nurses are great. Caregivers are friendly. Sometimes understaffed, hard to reach my friend by phone. Biggest issue: lack of communication; decisions made without my input. Overall, I'd recommend Charter Senior Living of Hazel Crest; my friend is happy.

"Mixed Experience with Activities and Food Quality at Assisted Living"
In terms of enjoyment, they offer plenty of social activities and enough space to move around. However, meals need better quality and more choices. We were promised outdoor walks in nice weather, but access is limited. Despite this, we recommend this community to other families.

"Welcoming and Caring Assisted Living with Healthy, Delicious Food"
My mom has stayed in a few places, but this one is really nice. The people who work there are super friendly, and the place is kept up well. The food is good, healthy, and tastes great. But what stands out the most is how the staff treats the residents with lots of care and love.

"Nice staff, but small, unhomey rooms."
Charter Senior Living was a nice facility with caring and attentive staff. However, the room we saw was very small; I’m not sure we could even fit her bed and dresser in there. To me, it didn’t feel homey at all. I know gray is the new color, but it just didn't feel bright and airy.

"Cleanliness Concerns in Dementia Community"
Keep an eye on things and don’t assume everything’s fine. My dad, in a secured dementia community, feels uncomfortable due to cleanliness issues.

"New Owners, Same Issues! Avoid this Assisted Living Facility!"
For the past five years, I've been dealing with how my mom's care at the assisted living facility got worse and worse. When she first moved in, the place was owned by Arden Courts and we liked it. But in just two years, it changed to Citadel Estates of Hazel Crest, without any explanation or introduction to the new owners. Over the last five years, there've been four different directors, none of whom bothered to introduce themselves. The place quickly went downhill under Citadel's management. It's gotten dirty, there are always new employees, and sometimes there aren't enough staff. The caregivers really do try, most of the time. But the management and nurses? Cold, unapproachable, and unhelpful. And you definitely won't get your money's worth. The food went from good to just okay. I had to be there a lot to make sure my mom wasn't being neglected or mistreated. This is not a place I'd ever suggest for anyone. And don't buy into their claims of 24-hour nursing care - it's simply not true. You might ask why I didn't find a different facility for my mom. The truth is, I don't trust any of them, and I can't take care of her myself because of my own health problems. I did my best for her before and during her time here.

"Dad's Happy Place"
My dad really likes living here. They take good care of him and plan fun things to do. The person who plans activities is especially good at it.

"A Welcoming Place"
The staff here are friendly and helpful. My brother enjoys the food, and the place is always clean. They even have fun activities, like a prom night with live entertainment. I’d recommend this community.

"Great Home Care Agency: Recommended for Excellent Service and Support"
My aunt is doing really well ever since she began getting help from this home care place. It's really good, and they've been super nice to us. Whenever we had a problem, they fixed it right away. I get along really well with the boss, she's awesome. Overall, it's been a good time, and I think other people should try this place too.

"Negligent Management Taints Assisted Living Facility!"
My dad stayed at Citadel of Hazel Crest for three months and urgent problems started showing up just days into his stay. An unfriendly dog, lack of cleanliness in both the center and with my dad, and unsuitable behavior by the staff were the primary issues. The final straw was when the facility's leader, English , openly said, "I approve their behavior and sometimes I forget I'm at work." This statement made me immediately take my dad out of Citadel. No word yet from the center's higher-ups. However, I must admit, the layout is pretty good for the residents. But this place desperately needs a major overhaul in management. The officials above the facility's director need to be more responsive.
