"“Prairie Lakes Neglectful: Look Elsewhere for Hospice Care”"
We were assured that Prairie Lakes could provide proper care for hospice patients who couldn't move or communicate their needs before we moved our beloved family member there. When we got there, the staff listened for hours to our hospice nurse and family explaining our loved one's needs. • On the first night, the staff tried to move our immobile family member to the bathroom but ended up accidentally dropping them. • The next afternoon, our family member was still left in the same dirty sheet that they arrived in. • Despite knowing our loved one couldn't eat on their own, the staff simply left food trays and seemed annoyed when asked to help feed them if we weren't around. • Medications were perpetually late and the staff needed constant reminders. On one occasion, they claimed that they already administered the medication, but since a family member was present at the time, we knew they hadn't. When we requested to see our family member’s chart to confirm the med was given, the staff got upset. • We even saw staff giving our loved one the wrong dosage of medication. • One aide told the nurse that a patient won't communicate their needs and they should stay there until they did. The nurse simply ignored this and left the patient alone in the room. For a week, we took shifts just to be with our loved one and help out. We even shared these problems with the staff and when we expressed our concerns, we were bluntly told by the Executive Director, that if we weren't happy with the care, we should find another facility. If your loved ones can't care for themselves and need extra help, I highly suggest that you search for another facility. Find one that's willing to communicate with you and understands the care your loved one deserves, especially near the end of their life. We eventually moved our loved one to a more compassionate, loving facility that met all their needs.

"Great Care and Friendly Staff at The Legacy"
I can compare different facilities because my mom had Alzheimer’s and was in another place before. We only had one option in our town since she was a Medicaid patient. If your parent is going into a memory care unit, be ready for some missing items because other patients like to “shop.” At The Legacy at Prairie Lakes, you can ask for a lock on your loved one’s door that can be opened from the inside. Laundry mix-ups happen everywhere, but The Legacy is small, so things can be found. Buy cotton clothes a bit bigger because the laundry is set very hot to keep things clean. You can order food from a menu if you don’t like the daily entree, which is great. The place is very clean and looks like a cozy hotel lobby. The staff are kind and seem to understand the needs of people with dementia. CNAs do the hard work like bed changing, diaper changing, tooth brushing, and bed making. They don’t get paid much, but many have hearts of gold. The Legacy is calm, pretty, and friendly. They always try to involve your loved one in activities. The food is great and they have their own chef. Skilled care is different. The rooms are small, but most patients aren’t very active. It seems short-staffed, but we’ve never had problems that weren’t fixed right away. No one will care about your loved one’s needs as much as you do, but the staff here are busy yet caring. It’s a safe place, but you still need to be an advocate. That’s true anywhere. My mom got good care, but the environment wasn’t as comforting, calm, or pretty as The Legacy.

"Caring, Comfortable, and Safe"
I’ve seen different assisted living facilities. My mom had Alzheimer’s and stayed at another place. In our town, there was only one option because she was on Medicaid. If your parent is going to a memory care unit, expect some missing items—other patients sometimes “shop.” At The Legacy at Prairie Lakes, you can ask for a lock on your loved one’s door that they can open from the inside. Laundry mix-ups happen everywhere, but at this small facility, things can be sorted out. Buy larger cotton clothes because they wash laundry on very hot settings for cleanliness. The best part? You can order food from a menu if you don’t like the daily entree. The facility is clean, like a hotel lobby but cozier. All the staff are kind and understand dementia patients’ needs. CNAs handle the tough tasks like changing beds, diapers, and brushing teeth. They don’t get paid much, but many have hearts of gold. The Legacy feels calm, pretty, and friendly. They involve your loved one in activities. Plus, the food is great—they even have their own chef. Now, skilled care is different. Rooms are small, and most patients aren’t very active. It seems a bit understaffed, but we’ve never had unresolved issues. Remember, no one cares about your loved one’s needs as much as you do. But this place has busy yet caring employees. It’s a safe environment, but you still need to advocate for your loved one. My mom received good care, but the environment wasn’t as comforting, calm, or visually pretty.

"Care Home Unable to Meet Basic Hygiene Needs"
My stepdad stayed at this care home for barely two weeks before my mom was informed that they wanted him gone. The reason? They said they didn't have enough staff members to handle his cleanliness requirements. Beyond this, the caretakers didn't even ensure he took a daily shower or put on fresh clothes. I would not recommend this place to anyone who even remotely needs personal care.

"Dissatisfied: Negligence and Lack of Care"
My stepfather stayed at this assisted living facility for barely two weeks before my mom was informed that they wanted him to leave because they were short-staffed and couldn't properly handle his hygiene needs. Moreover, they failed to ensure that he showered and changed his clothes every day. I wouldn't recommend this place to anyone who even slightly needs personal help.

"Disheartening Experience with Noblesville's Prairie Lakes Health Campus"
Based on our enduring relationship with Prairie Lakes Health Campus in Noblesville, IN, we've encountered a disappointingly inefficient and inexperienced administration. This disappointingly includes their Executive Director, D.O.N., Social Services Director, various unit managers, and office personnel. They've got a lot of degrees, certificates, and licensures, but those don't always guarantee good performance. Sadly, this is exactly what we've learned at Prairie Lakes. Since 2015, we've noticed failing healthcare procedures in various areas such as poor medication management, lack of personal cleanliness, misdiagnoses or failure to diagnose, and messy medical recordkeeping. On top of that, patient information frequently misreported. Care plans for the residents are created but not followed, and the social services department mostly stands against the patients and their families, rather than supporting them. And it doesn't end there; they don't have suitable activities for all residents, the weekend staffing is a continuous problem (which the facility denies, but see for yourself). We've also seen an increase in theft of personal belongings and Prairie Lakes does not respond appropriately when it happens. Rest assured, this review is based on actual facts, not rumors or false claims. While there might be positive reviews on their Facebook page, I doubt these reviewers have had a lasting experience with this place. The administration lacks empathy and is unqualified to provide long-term care. If you ask questions or go against their plans, you risk being labeled as a trouble maker and your family member may be discharged unfairly. I took on the responsibility of securing excellent long-term care for my parent, making sure they lived in a clean environment and were treated with respect until the end. Prairie Lakes promised all this when my parent was admitted, but they failed to deliver. I wouldn't personally recommend any nursing home unless absolutely necessary, and Prairie Lakes is far from an exception. Despite their slightly better rating on the Medicare ucomparenursinghome website, this establishment operates more like a glorified warehouse than anything else.

""Longtime family records poor management, care at Prairie Lakes Health Campus""
We had a long relationship with Prairie Lakes Health Campus in Noblesville, IN and we were not impressed. The management, including the Executive Director, D.O.N., Social Services Director, and other staff members, seemed inexperienced and inefficient despite their credentials and degrees. Just because they have paperwork doesn't mean they know their jobs. This was sadly true for Prairie Lakes. We saw a lot of problems starting in 2015. They didn't take good care of residents. They made mistakes with medicine and didn't pay attention to personal care. They didn't understand medical conditions or keep proper medical records. They would even lie about resident info. They didn't follow through with the resident care plan and didn't help families. They made life hard for residents and families alike. The activities they had weren't right for everyone. They had staffing problems on weekends but never admitted it. If you don't believe me, stop by and see for yourself during the evening shift or on the weekend. Stealing was a big problem in Prairie Lakes. When things got stolen, they wouldn't do what they were supposed to. Everything I'm saying is true. I'm not making up stories. I doubt that people who say nice things about them on their Facebook page really know them. The management had no sympathy and weren't fit to take care of others long time. If you or your loved one didn't fit with their plans, they would label you a troublemaker and try to get you out. I was supposed to find a good, clean place for my parent to live and be treated well until they passed. That's what Prairie Lakes promised, but they didn't deliver. We wouldn't recommend them or any other nursing home if you can avoid it. If you look up Prairie Lakes, you'll see they're just a fancy warehouse with a slightly better rating on the Medicare nursing home comparison site. But that doesn't mean anything when they don't care for people properly.

"Bogus Credentials: Poor Care and Negligence at Prairie Lakes"
We've had a long term experience with Prairie Lakes Health Campus Noblesville IN and were sadly disappointed. Their management team, including the Executive Director, D.O.N., Social Services Director, and others, though holding many degrees and certifications, were unskilled and unproficient. This became hugely apparent in 2015 when several major problems came to light. These include things like poor management of medications, inadequate personal care, incorrect diagnosis, and badly handled medical records. In addition, we found their resident care plan was not adhered to, and the Social Services department was not helpful. Activities were not suitable for all residents, and there was insufficient staff on weekends, despite the facility's denial. Theft of personal items was a big issue, and there was no proper response from Prairie Lakes when it happened. Our review is based on facts, not just assumptions. We can't say the same about the positive reviews Prairie Lakes gets on its Facebook page. It is our belief that the management lacks empathy and the necessary skills to provide long term care. If you speak up or question their system, you might be deemed a trouble maker with a risk of your loved one being discharged prematurely. When we chose Prairie Lakes for our loved one, we expected excellent long term care, clean surroundings, respect, and dignity, as they had promised. But they failed to deliver. We would not recommend using nursing homes unless it's a must. Make sure to explore other choices for your loved ones. Prairie Lakes is just a glorified warehouse with a slightly better rating on Medicare's comparison site, but this higher rating means nothing to the actual care they provide.

"Inconsistent Personnel and Subpar Care at Prairie Lakes"
Before moving my parent to Prairie Lakes, I visited many places. Prairie Lakes seemed like the best option for us based on the care services they offered. The nursing staff alongside the sales and marketing team did a decent job, convincing us to choose Prairie Lakes. However, while at Prairie Lakes, we noticed several personnel changes, including numerous executive directors, social service directors, and head nurses. This lack of stability affected the quality of care for our loved ones. Caregivers, nurse aides, and other staff members frequently quit due to these constant changes. There was no consistent care, leading to an unacceptable level of care for our parents. These negative experiences mean I can't rate this facility well. A place is only as good as its management, and Prairie Lakes' current management is dreadful. You can compare nursing homes on nursinghomecompare.gov, but the information might be outdated because of the delay in posting. Plus, all nursing homes should present their latest health department inspection report for guests to examine. Prairie Lakes has been criticized for not making this information easily accessible. You can find inspection reports on the Medicare.gov website.

"Prairie Lakes: A Poor Experience Due to Unstable Management"
Before deciding on Prairie Lakes for my parent's care, we visited several spots. This place seemed best based on the services offered. The facility's nursing staff, along with their sales and marketing team, convinced us to choose Prairie Lakes. However, while we were there, the facility had constant staff changes - four Executive Directors, multiple Social Service Directors, and an uncountable number of Nursing Directors. This instability trickled down to the caregiver level and caused frequent shifts in nursing staff, aids, and other employees. The constant change led to inconsistent care, which was unacceptable for our parents. I can't recommend this facility due to the issues we had during our stay. The quality of a facility is based on its management, and their management was severely lacking. You can check stats for all nursing homes at nursinghomecompare.gov, but keep in mind these numbers might be behind as they can take a few months to post. All nursing homes should have an up-to-date health inspection report accessible for guests, but Prairie Lakes had been called out for not making this information available. You can find inspection reports on the Medicare.gov website as well.

"Small rooms, odd care setup; inconvenient for loved one."
We checked out this place for our family member's care. It seemed nice, staff was friendly, but rooms were too small. Wish they listed room sizes online. They suggested an odd care setup. Our loved one couldn't access his room until evening.

"Good Skilled Nursing Facility with a Few Drawbacks"
I thought it was good for skilled nursing, even though there was a slight smell not as bad as Riverwalk Village. My husband’s grandma stayed there a few years ago, and everyone who helped her seemed really pleased. Unfortunately, they didn’t take Medicaid, and their waiting list was quite long. We needed something more immediate.
