"Assisted Living Facility Review: Layout and Dining Recommendable"
The rooms and layout are great, 4 out of 5. Before COVID, we ate meals with our loved one; food was average, 3 out of 5. We recommend this community to others.

"Sub-standard Service: Poor Food, Ignored Calls, & No Provided Transportation"
They don't offer rides, they just call TARC which isn't free and the resident has to pick up the tab. The grub here is awful. They don't cater to special dietary needs. The head honcho never gets back to you on the phone. When you hit the emergency call button, they take forever to respond, sometimes it's 15 minutes or even longer. They don't regularly test the batteries in these buttons. They haven't even bothered to hire a maintenance guy. There's also no one in charge of planning activities. The staff team isn't big enough and people quit all the time.

"Disappointing Management and Services at Assisted Living Facility"
The management keeps changing at this assisted living facility. In the past five years, they've been through four different managers. The owner doesn't live nearby, but instead in California and Mexico. The facility promised lots of activities for my mom when she moved in, like transport and outings, but they've all failed to happen. The food they serve doesn't have any variety and is mostly carbohydrates. Fresh food is rarely available. Also, laundry is all washed together without separating different types of clothing. The cleanliness of the rooms isn't good either. The floors are made of concrete and covered with indoor/outdoor carpet, making it hard and dangerous if a resident were to fall. They have a separate wing for Alzheimer's patients, but I dread to think about its condition since the main facility is poorly managed.

"“Impressive Improvements and Excellent Staff!”"
I visit my aunt at your place, and I’ve seen lots of changes lately. The whole vibe feels different now. We were thinking of moving her, but not anymore. The new director had a long chat with our family—over 45 minutes! And now we’ve got a tour set up for my uncle. Great job!

I visit my aunt at your facility and have noticed many improvements in the last few weeks. It feels like a completely different place. We were thinking about moving her, but not anymore. The new director talked with our family for over 45 minutes. Now we have a tour set up for my uncle. Thank you, & staff, for an excellent job. Sincerely,

"Avoid this facility – poor staff commitment and high fall rates"
This is the absolute worst facility in Hardin County. Just take a look at the news articles published on August 21st. Assistance is a rare commodity, as it seems like helpers are never around when a resident needs them. Oddly enough, the staff can often be found outside enjoying a smoke break while the elderly are left unattended. The number of times the staff line-up changes is mind-boggling. I implore you to check with our local county station for the alarmingly high number of ambulance calls - mostly due to falls - just this past summer from this facility. The owner seems absolutely clueless about what's happening here - which may be because they live all the way in California. I have so much more I could divulge about this horrid place. I deeply regret ever deciding to place my mom there - it was an awful mistake.

"Thank You, Bluegrass!"
My sister and I were so happy to get a room at Bluegrass Assisted Living for our mom. The staff was very friendly and really cared about her. For the first time in a long time, we could relax and not worry about her. We hoped she could stay at Bluegrass for many years, but sadly, she got worse quickly and had to move to a nursing home. Based on our experience, we would recommend Bluegrass Assisted Living to anyone looking for a nice, caring place at a good price.

"The facility cares"
We’re happy that my Dad is doing well at this facility. We realized quickly that we couldn’t take care of him at home. Bluegrass Assisted Living is the best choice for the money and the staff. However, I think Blue Grass could use some common sense in their approach. The TV mounted over the couch is a mistake. How can you sit on the couch and stare at a blank wall with the TV behind you? A better sign-out sheet, with names and times, would prevent confusion when people are unaccounted for. Overall, it’s a good facility, and I’ve enjoyed working with the management staff.

"Staffing Concerns in Pleasant Assisted Living Facility"
This place is pretty good overall. It looks nice and stays clean. The workers are friendly and helpful. But there aren't enough of them. Sometimes it feels like they care more about money than taking care of us properly. Especially at night and on weekends, they don't have enough people. Also, the emergency necklaces don't always work right.

"Limited off-site activities but reasonable rates, nice facilities."
The assisted living facility has good amenities, but they only offer one off-site activity per month. It has fewer units, but the cost per room is quite reasonable. They also provide Alzheimer's units at an affordable rate. Plus, it's less than five minutes from my home.
