Oak Park Village at Slidell

"Mixed experience at Oak Park Village; staff helpful, room for improvement."
The staff at Oak Park Village are friendly and responsive. They've always been helpful when I have questions or need assistance with issues. However, I've had difficulty obtaining information about how often they check in on my mom, who resides in the memory care wing. Unlike the assisted living area, there's no visible log of when they assist her with bathroom trips or safety checks, which concerns me. Housekeeping is average; while trash is consistently emptied, the bathroom only receives a quick wipe down. I've had to sweep her floor myself multiple times due to crumbs or other small debris. Regarding food quality, I can't personally attest, but it appears appealing. Although my mom dislikes it, residents seem engaged in activities provided by Oak Park. The move-in process was straightforward, aided by helpful administrative staff. Overall, Oak Park Village is a slightly above-average facility for its type, with happy assisted living residents. It's comparable in cost to similar facilities, making it a reasonable choice if private paid care is within budget.

"Communication and Food Quality Issues at Assisted Living Facility"
I wouldn't recommend this community for loved ones. We faced communication issues, often needing multiple attempts to reach nurses or our loved one. The food was consistently served cold with dry, bland taste.

"Assisted Living Facility Provides Excellent Care and Activities"
The workers always kept the place really clean and were super nice. Oak Park Village was definitely worth the cost for the care they give. They had fun things like bingo all the time, and they often took the residents out for trips. My dad really liked the staff there a lot, and they treated him really nicely.

"A Great Assisted Living Facility!"
My Dad moved here three weeks ago. At first, he didn't want to stay. But after visiting him every day for a couple of weeks, he settled in and now really likes it. He's making friends, joining activities, and loving bingo. The staff is friendly and helpful. I feel comfortable leaving him here. He wanted independence with help available when needed. The food, prepared by a professional chef, is great—he hasn't missed a meal! It's the best place for him, and I highly recommend it. The small community has an inviting atmosphere, and my Dad feels at home.

I moved my dad here and after visiting him every day for a couple of weeks he has settled in and really likes it. He loves the activities, particularly Bingo, and has made friends. The staff has been great, very friendly and extremely helpful. I now feel comfortable leaving him there with no worries. He wanted to be independent but have help available when he needs it. He loves the food which is prepared by a professional chef. I would recommend it highly as a great facility.

"Frustrating Care Mistakes and Miscommunications at Assisted Living Facility"
My mom stayed at this care center for nearly six weeks, but following two hospital visits, we moved her out as quickly as we could. I stressed two important things they needed to remember: she can't tolerate milk, and she needs her food chopped into small pieces. Neither of these requirements was met. The staff here simply aren't given enough training. A variety of additional issues came up. For example, she was supposed to take a specific medication to help her sleep at night, but it was given to her in the morning instead. They never made efforts to get her to leave her room, which might have been caused by the incorrect medication timings. She also needs three different blood pressure pills every morning, but they only gave her two, and at night. She developed a rash which wasn't communicated to me, and now her skin is close to becoming broken in that area. All the activities listed on their calendar are simply not true, they rarely ever take place. I wouldn't advise anyone to choose this facility.

"Avoid This Facility - Lack of Proper Care and Communication"
My mom stayed in this facility for 6 weeks, during which she had to visit the hospital twice and we finally decided to remove her as soon as we could. I had explicitly told them that she was not supposed to have milk and she required her food to be chopped into smaller pieces. These instructions were overlooked. The helpers there lack the necessary training. There were numerous [Removed] like the bedtime medication she was supposed to take was administered in the morning instead. They didn't promote an active lifestyle, didn't encourage her to leave her room, and I suspect this could've been a result of incorrectly timed medication. Furthermore, she was prescribed three different blood pressure pills to be taken in the morning, but two were given to her at night. She developed a rash which they failed to inform me about; now her skin is in poor condition. The engaging activities shown on the schedule were just fabrications; none of them actually came to pass. I don't think this place is recommendable.

"Mom’s transition eased by helpful staff and new friendships."
My mom moved to Live Oak about two weeks ago. It was a major adjustment since she didn’t move willingly, but the entire Live Oak staff has been incredibly helpful in easing her through this tough transition—even helping her adopt a kitty companion. The facility is cheerfully decorated and offers many amenities for residents and their families. Mom was able to furnish her own apartment and personalize her ‘front porch.’ The meals are tasty, and the dining room allows residents to meet each other, helping Mom develop new friendships. What a difference from being alone and overwhelmed at home to being in a caring environment with friends! I look forward to enhancing this review in the coming months.

"Two Weeks - and Live Oak Has Been Amazing"
My mom moved to Live Oak about two weeks ago. It was a big adjustment since she didn't want to move. But the whole Live Oak staff has been amazing, helping her through this tough time—even helping her get a kitty companion. The facility is cheerfully decorated and has lots of amenities for residents and their families. My mom got to furnish her own apartment and even decorate her "front porch." The meals are tasty, and the dining room gives residents a chance to meet others. My mom is already making friends, which is a big change from being alone and overwhelmed in her own home. I look forward to updating this review in the coming months.

"Needs more exercises for mobile residents. Overall, good service."
My dad moves better than others here. Exercises suit less mobile. More variety for active folks would be great.

"Assisted Living Turns Mom's Experience Around in Three Chances"
It's a clean place, and everyone is friendly. The first time my mom was there, it was during a lockdown, and she escaped, which wasn't good. They made us get sitters around the clock because she kept pulling alarms. Her medication wasn't right, so they suggested the sitters, but then she locked them out and escaped through a window. Live Oak took her back for a second chance. The second time, she lasted a week before needing to go back to the hospital, but they welcomed her back again. Now that she's on proper medications, they've released her from lockdown, and she's enjoying activities like bingo, has her dog back, loves the food, and they even pamper her dog. It's such a relief not getting daily calls about her behavior. I appreciate the small community, and especially now that they let her socialize more. The progress reports and monthly assessments are a big plus for me.
