"Friendly Assisted Living with a Medicaid Option"
Good things: It’s clean, staff is friendly, food is nice, and the price is reasonable. Not-so-good things: Sometimes there aren’t enough staff; services can be a bit disorganized; and there aren’t many activities for residents—Bingo isn’t cutting it anymore. They do accept the Medicaid Waiver, which is great because not all places do.

"Poor Communication and Boredom Plague Assisted Living Facility"
This place started out okay. But, a bunch of management changes made it go downhill. Our family member fell, wound up in the hospital, even got sick with Covid - and no one told us a thing. Raise your hand if you think that's a bad sign. Good luck trying to call this place. They gave us a bunch of phone numbers but no one ever picks up. So, if you want to know how your family member's doing, you're out of luck. In the old days, there were things for the folks to do. But now, that's not the case. Everyone just sits there in the big room, bored out of their skulls, every day. The only bright side? The food's fine. Seems like they have a good cook. But with everything else, I'd tell you to think twice before letting someone you care about stay here.

"Abysmal Care and Lack of Organization in Assisted Living Facility"
My mom started living here and things started to go downhill when a new director came in. The staff keeps changing and the whole place lacks organization. It's like they don't even know when their residents have their last bath, or when they change their clothes or bed linens. I patiently waited for months hoping to see some change. Every time I visit my mom, I end up cleaning her room. I had to scrub her sink and toilet just to make it presentable. She needs a seat riser because of a knee surgery and I found myself cleaning it in the shower. I found over 20 plastic cups, dust, shoes, and socks hidden under her bed. Even her medicine cabinet is rusted out. One day, I found her toilet tank's lid smashed into pieces, covering the entire bathroom and three out of the four bulbs in her vanity were burned out. They replaced the lid but it doesn't even fit right. And, I noticed my mom's clothes never really look dirty as if they never get changed.

"Subpar Experience at Crystal Creek Assisted Living Facility"
I didn't like Crystal Creek much. It cost a lot and the rooms were small. The people living there were much older than my loved one and needed more help.

"Mixed Feelings: Friendly Staff, Dated Atmosphere at Crystal Creek"
The staff were really nice when we visited. We didn't pick Crystal Creek because it felt old and like a hospital. It was less expensive, but we chose a place that felt like a nice hotel since we could afford it.

I didn't like this place at all. It seemed poorly run, and the atmosphere wasn't what I wanted. I scheduled a tour online, but when I arrived, they didn't know I was coming, which was frustrating.

"Affordable care with family involvement; dated facilities and small rooms."
This community is for those with a budget. The care seems really good. They handle their own tests for UTI and don’t always call 911 if someone faints a lot. They talk to the family to decide what to do in non-emergencies. But the buildings are old and the rooms are small compared to fancier places.

The assisted living facility has a spacious layout with four buildings for different care levels. It's clean, well-organized, and the rooms are a good size for dementia residents.

"Great place"
Crystal Creek is a really nice place. It's well managed and treats its residents very well. Plus, it's a great deal. I gave it 4 stars for cleanliness because the decorations are old, but they do a great job keeping everything clean. I'm happy my mom lives there.

My family member moved into this place, and we're happy! The people who work here are really nice, and I trust them to take care of her. She likes the food and has fun doing things with the other people who live there. We're happy with everything and think others would be too!

"Caring staff makes up for dated decor"
Crystal Creek takes great care of my mother. I'm really happy with the staff and the facilities. Some people might not like the decor since it's a bit old-fashioned and not as "fresh" as other places. But the staff's attentiveness more than makes up for it.

"Owner's Failings Create Nightmare Conditions in Facility"
My mom stayed here for four years. After two years, the place got sold to new owners and everything just went downhill. They give residents Risperadol, a heavy-duty drug that makes people super sleepy. It felt like they did this because people are easier to take care of when they're almost knocked out. But this drug caused a bunch of residents to trip and hurt themselves. Besides, Risperadol isn't even safe for people with dementia. When we found out they were giving it to my mom, we put a stop to it right away. The way they treated people was just plain wrong. My mom would be left for hours without being taken to the bathroom, and she'd end up sitting in wet or dirty adult diapers. It's not fair to her or anyone! I don't hold the nurse aids responsible because they had too many people to care for and were barely getting paid. But I've got plenty more things to say about how bad the place was, and all the blame should be thrown at the owner.

"Decent Tour, Not Perfect Match."
I went on a tour of this place for my dad. The staff were on time and answered all our questions. It wasn't the perfect fit in the end.

"Approachable staff!"
My family member really likes the staff at the assisted living facility! They take good care of their needs and listen to them. The administrative staff is friendly too. The only downside is that my family member doesn’t get to interact with others much due to their higher cognition level. Overall, their stay is going great!”

"Decent Assisted Living: Worth Considering"
Mom had a nice break at this place. The workers were alright. We had a few problems, but they fixed them. Seems like a nice spot. Mom enjoyed the meals. Providing this service seems tough, but they manage it decently. I suggest checking it out.

"“Two Months of Heartless Neglect at Crystal Creek""
In 2017, my family trusted this place to take care of our dad, who was suffering from Alzheimer's disease. His health got worse during the two months that he was there. He even fell and ended up needing stitches – not a single staff member saw it happen, and we have no clue how long he was left on the floor without help. We okayed medication to keep him safe, specifically for his sundowning symptoms at night. However, staff never gave it to him. They also couldn't manage to give him a proper bath, and he went weeks without basic cleanliness. We, his family, were the ones shaving him, cutting his hair, and trimming his nails. He began to stay alone in his room, sleep longer, and eat less. It was said that he got physical with another patient – again without any staff witnessing the event. They made him undergo a psychiatric evaluation at the hospital. The hospital didn't want to keep him, citing his severe Alzheimer's and behavioral issues. He was sent back without any prescriptions for his mood or behavior. Crystal Creek asked us to hire personal caregivers for him to return. When we questioned this, they claimed his care was supposed to be all-inclusive. Without telling us, they requested a second psychiatric evaluation at a different hospital. We didn't even know until we visited him one day, and the staff told us that he had already been taken away in the ambulance. The ER team at the second hospital didn't know about his previous evaluation or the Haldol injection he received. I went to Crystal Creek the day after his second hospital admission and they said he could come back, even without personal caregivers. They also said he'd be placed in another building. While in their care, he was diagnosed with a urinary tract infection (even though he barely used the adult diapers we provided) and needed to stay in the hospital until he got better. Crystal Creek along with the hospital planned a return date. The hospital discharge memo labeled him as a late-stage Alzheimer's patient with behavior issues. Even though no prescriptions for his behavior were given, they let him go. On his supposed return day, my brother went to gather his clothes and shoes, but was informed that the owner wouldn't let dad back. We were forced to find another place for him. The second hospital agreed to provide care until then. It took seventeen nights at the hospital while Crystal Creek kept assuring us he could come back. Crystal Creek completely failed to provide decent care or to communicate about his needs and conditions. They didn't coordinate his care plans and put him in danger. Even though they evaluated him before admitting him, they showed zero understanding or knowledge about caring for an Alzheimer’s patient. They displayed complete disrespect and lack of compassion towards him and us. When he was admitted to Crystal Creek, dad could walk without help, use the toilet with reminders, mingle, eat well, and participate in activities. But his health and abilities took a nosedive in the two months there, and now he needs hospice care.

"A Great Place for My Aunt"
My aunt is getting really good care here. I'm happy she moved to this place. They have a great staff, and they've been really nice to work with. She's joined some of the activities they have. Everything has turned out great, and I would recommend this place.

"Safe and Friendly Place for Mom, Satisfying Experience Overall"
My mom is doing okay here. It's a nice place and she seems happy. Everyone is friendly and nice. I feel good knowing she's safe and well taken care of. They're giving her good care, and I'm happy with her being here. We've had a good time here.

"Lack of Care and Cleanliness at Assisted Living Home!!"
Whenever I visited my relative, they were always unclean. I even bought them a new toothbrush, but I found out it was never used. Throughout the day, when the supervisors were present, the staff seemed busy assisting the residents. But during evenings when they were unsupervised, they would simply lounge around, glued to their TV screens and mobile phones. The facility has an outdoor porch, which is consistently messy.

"Outdated Assisted Living Facility Lacks Interaction and Cleanliness"
The residence we visited isn't as modern as the others we've seen. Despite booking a meeting time, they seemed unprepared for our arrival. We saw one of their vacant rooms, which had a really bad smell. Additionally, it didn't seem like there was much communication happening between the people living there and the staff. She was the person showing us around and gave us some details about the place. She was really nice.

This place is designed for Alzheimer's patients. The cost is reasonable, and it's clean with friendly staff. The rooms are spacious and wheelchair-friendly.
