"Avoid This Facility: Poor Hygiene Standards, Unkind and Inactive Staff"
You should definitely steer clear of this facility. When we visited our loved one, we saw a young staff member just sitting at a table in front of the fridge with her head on the table. I expected her to be interacting with the residents or maybe cleaning but nope. Even though they claim the rooms are cleaned weekly, that's not really the case. The bathrooms are outright dirty, and it's clear nobody has vacuumed the carpets. The staff in charge of activities is just bad. Not only that, but we've seen staff members being unkind to residents who try to leave through the locked door. Also noticed staff hanging out in front of the facility, and they all smelled like smoke. It was gross.

"Mixed Care Facility Falls Short in Food and Entertainment Options"
This place is okay because it offers different types of care. My husband needs help remembering things, so it's good for him to be around others who can remember better. But he's not happy with the rules there. The food isn't good either. Mealtime is sometimes the only good thing. They don't think about different people's needs when it comes to food. I've told the boss and the workers, but nothing has changed. There's also no one planning fun things to do, and we haven't gone anywhere since my husband came here.

"Mixed Memory Care and Nursing Home; Didn't Like the Experience"
I didn't like this place because they put people with memory issues together with those in the nursing home. I'm not sure if it's because they don't have enough staff, but I really didn't like that.

"Positive Experience: Clean, Helpful Staff, Dad Enjoyed Exercise Program"
My dad is getting used to it and doing well here. The staff is helpful and kind to him, and the place is always clean. He enjoyed their exercise program for a while. Overall, we're pleased with this community.

"Gracewood Assisted Living: Mixed Experience with Room for Improvement"
My mom has been at Gracewood for about 3 weeks. The nurse I worked with was great, but unfortunately, the day after my mother arrived was her last day. That caused confusion about my mom’s care, missed medications, and a failure to change a dressing. The meal trays sat in my mom’s room for hours, and she appeared to be left alone in the same chair for too long. The alert system is a pull cord on the bed, so if she’s in her chair, she can’t let anyone know she needs help. We talked to the house manager and interim nurse about fixing that, but they never did. My daughter finally fixed it last week. Overall, I think they care about the patients, but they might be understaffed. The staff we’ve worked with are nice, and the facility is new and clean. Now that Hospice is visiting my mom, her care has improved. I like the visiting physician a lot. The food isn’t great for someone on a soft diet; my mom even showed them how to make mashed potatoes, which is her main diet.”

"New facility with friendly staff and good care."
This place is pretty new, so they're still figuring some things out. It looks nice, and they do a great job keeping it clean. The staff has been friendly and responsive to my worries. They offer fun activities for the residents, and the food satisfies her. I feel comfortable with the care they provide. Overall, things are going well.
