"Heartbreakingly Poor Care at Forest Glen Assisted Living Facility"
My mom stayed at Forest Glen following her strokes and for short-term care. Honestly, this place does a terrible job at being a care facility. We, as her family, had to make sure they met her most basic needs. She always smelled like urine and I had to clean and change her every time I visited, which had to be everyday due to the lack of care. I consistently had to remind them to put water near her bed, but even then, they’d set it too far for her to reach. The staff also needed frequent reminders to give her a bath. She often came back from short-term visits with bruises, sores, or rashes. They even dropped her a couple of times and never followed our instructions to always have two persons help her with her wheelchair and bathroom needs. One time, they almost tore her finger off during a transfer, landing her in the ER. Despite several discussions with management, things would only slightly improve for a brief period before the dismal care continued. They were just as bad with my aunt who stayed there shortly before passing away. I would definitely advise against sending a loved one there. It's heart-wrenching. The administrators and staff should feel embarrassed about their treatment towards elderly people. It just kills me.

"Negligent Senior Care at Springfield Ohio Forest Glen Facility"
My father was in the memory care section of Springfield Ohio's Forest Glen for a short period, barely over a month. In that small space of time, he got a lot of painful sores on his back, shoulders, feet and sacrum - these were from being strapped into a wheelchair each day and left in one position. There were days when they just left him in bed. Once, we visited at 4:30 pm and they had left him naked in bed full of urine, for anyone to see. He hadn't had food or drink all day. Shockingly, he lost much weight during his stay. Dr. wouldn't give him any painkillers, not even the ones he'd taken for years for his old hip pain. As for end-of-life care, the doctor said no to Hospice support, even though my dad made the cut. Please reconsider if you are thinking of sending a loved one here. Management knew all this but did nothing. My father had to suffer a lot of pain from those sores in his last month. This place is awful!

"Hygiene Concerns and Undiagnosed Issues at Assisted Living Facility"
My family member stayed at this place, but we left because of hygiene problems. They were supposed to help her bathe, but if she said no, they wouldn't do it, even though she wasn't able to decide properly. After we moved her, we found out about other problems they didn't notice.
