"Terribly Managed Facility, Neglected Patients, and No Basic Supplies"
This place is really bad. They've got mold all over. My relative's room is always dirty. They don't even bother cleaning up after the elderly here, especially when it comes to their bathrooms. I've visited a couple of times and saw aides shouting at folks with dementia for a long time. They don't help patients wash their hands before their meals. Countless times, I've showed up to find no nurse or aide in sight. They don't even have toilet paper for the elderly during weekends. If you don't mind any of these, then this is the perfect place for you.

"Neglect and Communication Issues at Assisted Living Facility"
During the two weeks at the assisted living facility, they didn't change the dressing for the LVAD patient's driveline. The patient had to keep asking for their medication. The third shift was the worst—they ignored the patient on the first night. The OT/PT staff was great, though.

I visited HPHC a few times this year. The place is clean. The staff are friendly and talkative. They kept me updated about my mom's health. However, my mom lost many personal things. Some were stolen (like clothes and money) or thrown away (like dentures). That's the only issue that stops HPHC from being excellent.

"Decent Facility, Inconsistent Service Quality"
Over a year back, my grandma started living there. It’s a tidy place. Most nurses are super nice. We’re really happy when certain nurses are working because they’re super kind and great. But with some, we don’t feel as calm since we’re not too familiar with them. Sometimes, we gotta remind them to clean the room or get toilet paper, and we’ve even had to do it ourselves. The folks at the front desk can be a bit grumpy and not so helpful. Also, some stuff they said they’d do at the start didn’t happen. I’d say it’s like getting a “C” in school.

The physical and occupational therapy are okay, but I can’t believe they actually consider microwaving food as cooking. More concerning is how the medical staff informed us of my husband's dangerously high sugar levels, saying they were 800 when it should usually be around 350. If it were really 800 he'd be in serious danger! So, I brought all his necessities and he's been taking care of his health without them knowing. The meals during the weekend are especially concerning, they're just not properly cooked! To top it all off, they refuse to share his medical reports from physical and occupational therapy with me. They say due to it being a different network... which is complete nonsense!

The staff at Hyde Park Health Center addressed all my concerns. When I arrived, they explained what to expect and how to raise any concerns about my care. The therapy department was really helpful in getting me back to doing things on my own.

"Disappointed with Facility’s Decline in Quality and Care"
About 7 or 8 years back, this place was super good. They had awesome doctors and staff who knew their stuff. But lately, it’s just not the same. The helpers here don’t act like they’re your pals, and they’re not doing their jobs right. The bosses don’t fix problems the way they should, or they just ignore them. I’ve even had to get new stuff because my old things got broken or taken. To put it simply, I’m not happy with how things are going here.

"Neglect and Mismanagement: A Heartbreaking Account of an Assisted Living Facility"
I'm writing this as someone with experience in multiple nursing homes and medical environments... I strongly advise not to place your beloved family member in this place. It seems horribly managed and there's a conspicuous lack of communication. During our visit, we came across two nurses and one aide. The aide was nice and helpful, but the floor nurse acted bothered when we asked for details about our family member's care. The head nurse was condescending in how she spoke to us about the care plan. When she realized she couldn't pull one over on us, she became irritated and started to use harsh words. It seems to me that the staff resort to intimidation to manipulate situations. I don't believe she anticipated our vigerous response. The condition of the center is really bad and needs immediate attention. The rooms are cramped and dirty, with dust on window sills and curtains. The hallways stink of urine, there's rust around the room sink, a panel is coming loose near it, the light above it is broken, and bed covers are stained. There was a recliner in the room, but who knows where it came from or if it's even clean. If admission happens over a weekend, it appears there's not enough staff to properly welcome the new resident. Our family member didn't receive a room phone until the next morning after being there from 5pm the previous day. They were alert and capable, so it wasn't a medical issue. We requested one, but it kept being forgotten. There was a meal menu with options that our loved one wasn't allowed to consume, but it was sitting on the bedside table. When confronted, the nurse said she didn't know much about our family member and provided us with some printouts...this is when things took a turn for the worse. Do yourself a favor and avoid this headache: look for other facilities with pre-certification recommendations.

"Mixed Experience: Great Care, Poor Food Quality, Dim Environment"
This place is pretty good overall. The people taking care of my mom are nice and do a good job. But, the food isn't very good, according to my mom and sister. Sometimes it's not even warm! The area where my mom stays feels a bit gloomy and dark, which isn't very nice. However, despite all that, my mom seems happy, and that's what really matters.
