"Nice place, but care and communication need work"
Walking into this place felt really good. It was clean and looked better than any other place we saw. But, the care from the staff wasn't what we wanted. We also had problems with the management communicating and delivering on promises. I still think this could be a good place for the right person. The looks and amenities were great.

It's overall pretty good, but the frequent staff changes suck because the new ones have to learn about the residents and their needs from scratch.

If there was a way to give this place negative stars, I'd do it in a heartbeat! My family member was transferred here from a local hospital after a complicated back surgery and was unable to take a shower for almost a week. We've caught the staff, including aids and nurses, telling lies to each other. They often 'pass the buck' here, a lot. They love to say, "I'll be right back," but if you see them again that day, consider yourself fortunate! It's typical for them to be 90 minutes late dispensing essential and pain medications - which is totally unacceptable! There have been several instances where my loved one has had to wait up to 45 minutes after calling for assistance before someone comes to check on them. Some nurses seem clueless (are they even fully certified?) - they actually told us that it was alright to combine Norco/vicodin with a muscle relaxer. This incorrect advice almost caused a medical emergency when my loved one's blood pressure dropped to dangerously low levels! One nurse's aide falsely claimed that my family member refused to shower, which is a flat out lie. Plus, they were once told they were behind two other people on a list for a shower, even though we could clearly see this wasn't the case. Additionally, the heating system in the rooms is messed up - it's been blowing out hot air in the middle of summer. Despite complaining, nothing has been done about this in over 72 hours. Is the maintenance team located out of state? Once, a nurse's aide took my loved one's ginger ale to pour it over ice and never returned that night. And, the food delivery system here is appalling. They've left food trays out from 6:20pm to 4:30AM. Believe me, there's so much more I could say, I don't even know where to start. So, I'm officially lodging a complaint with the Ohio Department of Health's Provider and Consumer Services Unit. Do NOT send your loved ones here unless you want them to suffer neglect, dishonesty, invisibility, and mistreatment. To prove my point, there was a case of a resident being strapped to a wheelchair here as well as an unsupervised individual who drowned in the pond in front of the facility. Both of these are factual incidents!

"Horrifying Experience at Assisted Living Facility"
Our father was discovered on the floor, bare and freezing, after just two days at the facility. He even suffered injuries when he fell. This place is absolutely terrible. I can't give them a good score in any area.

"Assisted Living Community: Well-Maintained with Limited Activities, Suitable Choice"
I visited this community for my mom's needs. The tour was good, and it's a pleasant place. The community is well maintained with neat surroundings. Staff were professional and informative. However, there aren't many activities for residents. Overall, it's a nice option for the right person.

"Friendly staff, clean facility, but food and activities lack"
Mom is still getting used to living at St. Clare Commons. She doesn’t like the food there, and there aren’t many activities in the memory care unit. The staff are friendly, answer our questions, and are professional. The prices are very reasonable for private pay, and I would recommend this place. They keep the community clean.
