"Great Choice for Memory Care"
I think Faye Wright Senior Living is a good place for Memory Care for my mom. The workers are really attentive and keep me informed about my mom. Sometimes on weekends, they could communicate a little better, but they're getting better at that. The cleaning staff are nice and do a good job. The food looks really good and tastes good too.

"Clean, friendly assisted living with shared apartments."
I visited this place, and I enjoyed it. It's super clean, and the staff is nice. Our guide knew a lot. They only had shared apartments, which didn't suit us, but overall, it was pleasant!

"Great Tour, Nice Community, but Location Made Me Choose Elsewhere"
The tour was great, and the community was very nice. It was hard to decide, and this place clearly focused on the needs of its residents. The location was a bit far, which influenced my choice. The staff were all skilled, and the tour was excellent.

"Mixed Feelings about Assisted Living Facility"
When we talk to the bosses, things do get better. But it's frustrating that we have to keep doing that. The place is clean and doesn't smell bad, which is great. The rooms are okay, though smaller than before. There's a nice outdoor spot with a patio. One staff member isn't nice, but others are great. Food isn't very tasty, but they're trying to improve. COVID has stopped activities for now, but they're trying.

"Clean Environment with Limited Activities; Scenic Drives for Residents"
The community still has its restrictions, but when we can visit and see it, it's very clean. The activities are somewhat limited, but they do take residents on scenic drives.

"Small, Personalized Assisted Living"
I discovered Farmington Square when my friend needed more than just assisted living. He'd been at another place for nearly 2 years, but when he got sick and needed extra help, they couldn't provide it. We explored other facilities and liked what F.S. had to offer. They have different care levels and separate buildings for each unit. The units are small, with only a few residents, so there's more one-on-one care. Initially, he needed more assistance, but as his health improved, he became more independent—even though he mostly uses a wheelchair. The place is clean, neat, and inviting, and the food is good, although there's only one main meal choice (with sandwiches as substitutes). I recommend checking it out and comparing before making your decision.

"Friendly Staff and Personalized Care at Farmington Square - Salem"
The staff here are super friendly and quick to help with any needs. The smaller community is a much better fit for our mom. She knows it's not her house, but she's adjusted really well. Mom has made some friends and joins in some activities. It's interesting to see how the staff interact with residents who have different levels of dementia. With only 16 residents, they can give more attention to each person. My advice to family members is to stay in touch with the facility and your loved one so you know their needs and concerns. The staff should know who you are and that you care about your loved one's care. For our mom, Farmington Square - Salem has been a great choice.

"Farmington Square Unsatisfactory, Failed to Provide Needed Care"
My dad stays at this place. Right now, he's in the hospital. The state got a complaint about Farmington Square in Salem based on how bad my dad was when he ended up in hospital. We've spoken to Farmington about him not getting proper care. They promised to fix the problem, but they clearly haven't. I wouldn't push anyone to use this place. They have to do better in caring for and checking their residents. Being understaffed is not an excuse for failing to give people the care they need.

"Caring staff, responsive management, great care for mom."
The caregivers really care about the residents. Management and staff listen to any concerns we have about my mom’s care. We appreciate them for taking such good care of her.

"Unfit living conditions, unfit for family placement"
This facility is terrible, it's filthy and I wouldn't dare let any member of my family stay here.

"Facility Fails to Deliver Care for Dementia Patients"
At first, everyone seemed kind and welcoming. However, once I moved my wife into this care center, I quickly discovered a severe lack of adequate and skilled staff, all the way from the director to the frontline caregivers. Yes, my wife presented challenges because of her dementia, but instead of caring for her, they simply doubled her diapers, barely checked on her, and kept her isolated. I personally found her wearing pull ups soiled with feces four separate times in three months and had to clean her up myself. With care like this, I can’t help but feel this place shouldn't be licensed to handle cases of reverse personality dementia. I would strongly discourage anyone especially with long-term insurance from considering this place. They made promises they didn't keep and even claimed to have filed necessary paperwork for my refund, which they hadn’t. When things finally got sorted, it was only because I diligently monitored every step until I was finally reimbursed, three months later. I cannot endorse this facility unless they change their management and hire skilled, compassionate people to look after residents who often need assistance. It got so poor that I had no choice but to relocate my wife from this facility.

"Unprofessional Facility Neglects to Communicate; Forces Family to Relocate"
My folks moved into Farmington Square, and initially, we were pleased with this community. Over time, however, issues with care quality, staff shortage, and high turnover rates worsened. The final straw came on New Year's Eve involving my mom. She was taken to Salem Hospital around 2pm and we weren't made aware until 10:30pm, when the hospital called about her discharge. Since my father had passed, my mother, who has dementia, was left alone at the hospital. It was a shock to learn that none of us, siblings, were contacted by the community. It's horrifying that Farmington Square failed to notify us. I attempted to call [name removed], the Executive Director, numerous times with no success. After this incident, we made the choice to move mom to another memory care community, where we've been much happier. It feels like there's a serious lack of professionalism among the Farmington Square management team, particularly when it comes to communicating. It appears they lack compassion for their shortcomings and understanding for what families like ours are going through.

"Friendly Community for Mom"
I really like the friendly community of people. It's a smaller group of residents, which is exactly what I want for 'a place for Mom'.
