"Mixed Bag at Assisted Living Facility"
The assisted living facility has both good and bad aspects. It's not very clean, and we're not fans of the in-house doctors. However, we chose this community due to its affordable price, convenient location, and availability.

"Assisted Living Facility Review"
The people who work here were really nice. The food was good, but they didn't give big portions. It was sometimes hard to finish the paperwork for long-term care. My family member was told she could walk outside at first. As her memory got worse, they said she couldn't go out alone anymore. The one-bedroom apartments are very nice and have plenty of space for your stuff.

"Autumn Leaves Communication Lapse"
I visited my mom at Autumn Leaves She was exhausted and couldn’t stay awake, so I asked for help getting her to bed. I left around and later, a Hospice Nurse called to say Mom had “passed away.” I was shocked. When I spoke to the resident nurse (NP) and he explained that the night staff had alerted him because Mom was struggling to breathe. He called Hospice to check on her. The Hospice nurse arrived at 11 PM, examined Mom, and left. Later they called back and said she had passed away. Neither Hospice nor Autumn Leaves informed me or even hinted that Mom was dying. The Autumn Leaves Nurse (NP) said it wasn’t their responsibility to call me. But both companies should have contacted me. Autumn Leaves failed in notifying me about my mom’s decline and death.

"Frequent staff turnover, good location, clean, nice amenities."
There was frequent turnover of the staff and a lack of communication between staff members from shift to shift. This made it difficult for the residents and the families. We chose this community because it's close to my house, I know some of the other residents, it's diverse, and I liked the different amenities they offered. Also, it was clean and the halls were very pretty.

"Great place, but feels overpriced for unused extras."
This place was pretty good. It had lots of extras and food options. Our big worry was that we didn't need all those extras we had to pay for, making this place feel too pricey for us.

"Missed Meeting and Only Saw Lobby Area"
They didn't show up for my booked meeting. Though honestly, there might not have been enough time for them to prepare. We only got to see the lobby area.

"Could Be Better: Visit Before Deciding for Elderly Loved Ones"
I visited this place thinking it could be suitable for my mom. Unfortunately, it reminded me more of a nursing home than a warm, welcoming community. The staff seemed to lack information about their own facility. I noticed that the place didn't look modern and up-to-date. Also, it lacked that cozy, at-home vibe. I would recommend families to check out the place for themselves to see if it's a fit for their elderly relatives who need extra care.

"Lake View Retreat"
The Autumn Leaves is a lovely community, and I'm quite happy. The setup is nice, with my own living room, kitchen, and balcony. I enjoy a beautiful lake view from my room window. The meals are tasty, although I'd prefer butter over margarine. The friendly driver takes us shopping. Overall, it's an improvement from my previous community.

"Great Staff and Happy Residents in a Clean Facility"
I really liked how the staff interacted with the residents, but what mattered most to me was seeing how much the residents seemed to love their caregivers. The facility was clean, and the studios for the residents were big. I visited twice at different times of day and had the same great experience both times.

This is a wonderful place, and we were very happy with it. The staff is super friendly, the place is very clean, and the food is good. We were really pleased with everything, and the only reason we left is because our loved one got better and didn’t need this level of care anymore. We would recommend them to anyone.

"Great Place with Good Space, Views, and Outdoor Activities"
I really like this place and went back for another visit. It’s just a 30-minute drive from home. The closet space is nice and roomy. There’s a fantastic view, plus plenty of places to walk, be outside, and even do some gardening.
