"Concerns Over Medication Management at Orchard Park Assisted Living"
My brother has been at Orchard Park for several months. We're worried because he isn't receiving his medications as prescribed by the doctors. The on-site nurse has left, which is quite concerning. Hopefully, they'll find a replacement soon.

"Good community, but far and missing some amenities."
It’s a good community, but it was too far from family. They also didn’t have all the amenities we wanted.

"Pretty Lobby, Enthusiastic Staff, Small Resident Rooms"
We checked out Orchard Park at Southfork while looking for a place for my mom. The lobby looked nice and the tour guides were excited. But, the rooms for residents were tiny.

We visited Orchard Park at Southfork, and it was a nice place to see. It was clean and the people were friendly. The staff, the care they offer, and what you get for the price all seemed pretty good.

"Neglect and Cover-Up: Assisted Living Nightmare"
Where do I even start? I’ve been trying to reach the Regional Manager, but no call back. So, I’ll post here, hoping someone responds. My dad’s been there just over a month, yet I’ve seen more bruises on him during this short time than in his entire life. The LVN in charge lies to cover up. The director never calls back. We asked about the bruises, only to be told, “It’s internal; we can’t share details.” My dad fell in his room, found in urine and throw-up. How does this happen?! He had to be hospitalized for 2 nights. There’s so much wrong here—please don’t bring your loved ones.

"Delayed refunds and unclear referral bonuses sour facility experience."
The facility is nice. They treated my mother-in-law well. But after she passed, it's been over 2 1/2 months, and they still haven't given us our money back. Every time I call, they say they've processed it, but we haven't gotten a check yet. They promise to call back, but they never do. It's made me feel really disappointed with the facility. It shouldn't take this long to give us our money back. Also, if you refer someone, you don't get a check like they say. They only apply it to the resident's account. That's what happened with the person who referred us.

"Neglected Patients and Hidden Charges - A Disappointing Stay"
My wife stayed here and they barely paid her any attention. They definitely need more staff. Things were okay at first but quickly went downhill. They slapped charges on my bill for things we never used. Their rehab program brought a ton of problems. The only positives were the slightly lower cost, and a doctor or nurse visited once a week. I wouldn't advise anyone to use this place. When we were leaving, the door was left open for about 45 minutes with a siren blaring, and nobody even checked to see if it was a resident.

"Friendly staff, clean facility, good food, and daily activities"
The community has daily activities and friendly staff who know everyone. The caregivers interact well with residents. Communication with staff is good, and the food is tasty with healthy options. The place is clean and well-kept. I recommend this community.

"Mixed Feelings, But Worth a Visit"
This place is really nice with lots of cool stuff for the people who live there. The workers were super nice and they did a great job showing us around and telling us stuff. But, we didn't feel like it was as safe as we wanted for my brother-in-law. Still, it's worth taking a look!

"Not The Right Fit for Us, But Overall A Nice Facility"
Unfortunately, my mother-in-law was only at this community for a few weeks before she broke her hip. She's currently in rehab, and we're unsure if she'll return here. However, the facility is nice. The staff was kind and responsive when she fell. Overall, they treated her well. The community is well-maintained, and they do a good job with cleanliness. It wasn't the right fit for my mother-in-law and her needs, but it's still a nice place.

"Mom's Care"
I really appreciate this community for taking care of my mom. The staff is kind and pays attention to her needs. They should clean her room more often, though. The menu could use some small improvements, but they do offer activities for her to enjoy. Knowing that she's happy here makes me feel good. I'd recommend this community for good care.

"Inhumane Conditions and Neglect: Unkindness, Not Care"
Never ever consider this location for anyone you cherish. They get an absolute zero from me! Conditions are disgusting - rooms aren't cleaned, cockroaches crawl around, and the bathrooms reek with unwiped urine and feces. They even leave soiled bedsheets on beds! Personal belongings mysteriously disappear, and no one knows where they go. As for the so-called 'care,' it's nonexistent. The staff, uncertified and callous, fail to show any compassion. Those needing memory care are just huddled around the TV, supervised by a single caregiver. There's no time or consideration for their need for rest and individual attention. Except for meal times, there's a maximum of one or two caregivers seen around. Don't expect them to assist if asked; they'll just shrug you off, giving empty promises of finding help. All while contradicting their initial claim of an ever-present nurse during your tour. There are also no activities for engagement. Forget about baths - residents have to make do with a damp cloth on their faces. The sign might say 'Assisted Memory Care,' but truthfully, there is no 'assistance.' I firmly believe anyone would be better cared for alone in their homes than here. Pay attention when you hear an elderly resident apologize to staff for an unknown wrongdoing - that's an alarming sign! The facility lumps together those with dementia, who have hearing problems, and Alzheimer individuals who are completely disoriented. This mix offers no stimulating environment for dementia sufferers, leading to their silent suffering and incomprehension of the ill-treatment. I'm gravely worried about both mental and physical abuse. Oh, and with zero security cameras around, there's no way of knowing which staff members enter or leave rooms. When you find unexplained bruises and swelling on your loved one, no one has a clue of its cause. Of course, people here having memory problems won't be able to tell you either. Can you really trust this staff? Extra caregivers appear only during meal times, but they're mostly kitchen staff and a few from behind the locked doors. Toilet visits for residents happen seldom, and diaper changes occur only at shift changes. No one keeps track of what happens during different shifts, and staff has no clue why some patients can't walk or how to properly lift them. My blunt advice for making your decision? Look elsewhere!

"Tour and explore care options"

"Wouldn't Even Send My Dog Here"

"Deceptive First Impressions and Unprofessional Staff at Facility"
When I visited this place with my wife, we were shocked to find it in quite a poor state. Walking in, you'd think it's beautiful but once you see the rooms, there couldn't be a bigger letdown. They were so filthy and overrun with roaches, it was just unacceptable. To make things worse, one of the staff, whose name I won't mention, was hugely unprofessional. She talked about Victory Lakes instead of focusing on the conditions of the current facility. It's closer to our house but we weren't interested in that! What we wanted was to find a clean and suitable place. Her lack of professionalism and empathy only added to our disappointment.

"Beware: This Place Is Worse Than It Looks"
This is a terrible place to live, so do not even consider it. The management team is just awful, they never make an effort to communicate with the residents. On top of that, they are quite unpleasant and unkind. To make matters worse, this place is always understaffed due to employees constantly quitting. This company lacks competent leadership and organization from the upper management to the staff. On a weekly basis, new faces arrive and they're gone before you know it. If I had known how bad this place would be, I wouldn't have moved here in the first place. I moved in August, and by December, I received a letter stating my rent was going to increase. To say the least, this was neither anticipated nor justified. Fortunately, I will be moving somewhere else next month. Therefore, I strongly urge you to steer clear of this place. What you see on the surface is not necessarily a reflection of reality.

"Unreliable Staff and Unexpected Fees Taint Assisted Living Experience"
Believe me, stay away from this place. The staff here barely respond to your calls. They constantly swap employees like I change clothes. Almost everyone is friendly except for one person who works in the memory care section. Do not go to that side, because she is rude and untruthful. She only tells you what you want to hear just to get you there. Plus, they have all these hidden costs that you only get to see after mom settles in, then they hand you the real bill. Additionally, they increase your rent by 6 to 8 percent every year. This place is nothing but trouble.

""Watch Out: Unexpected Fee Hike at Assisted Living Facility""
Watch out for the fees at this assisted living facility. At first, they told us a certain cost for my dad's memory care. They stuck with that price for the first month. But the next month, the cost shot up by $320. We told them what our budget was, so this wasn't fair. We're now on the hunt for a new place. We're really upset about this situation. You should be cautious, too.

Thanks to A Place For Mom and Orchard Park for making sure my mom feels safe and well cared for.

"Brackets"
We first visited this assisted living facility for my 90-year-old father-in-law and initially, it appeared to be perfect. The aesthetics are appealing but sadly, the staff behavior left much to be desired. My father-in-law suffered a fall that resulted in a fractured hip and was provided with a wheelchair devoid of foot pads. This caused further discomfort as he had to raise his legs whenever he was wheeled around; otherwise, his legs would drop, causing a sudden stop and further strain on his injured hip. Despite his pain, the staff would command him to lift his legs which undoubtedly increased his agony. Subsequently, he was hospitalized due to severe pain and while returning, we specified that he was still in too much pain to be moved. However, the staff attempted to transfer him from his bed to the wheelchair post his return. This resulted in another fall, from which he sadly never recovered and passed away. Regrettably, the facility has refused to share the accident report with us, citing it as a violation of their company policy. Hence, we are left without a detailed understanding of what actually happened and how he fell. Disappointingly the director never made the effort to express his condolences about the loss. This sort of indifference towards their residents speaks volumes about their primary interest - money."

"Few Hitches at Southfork But Staff Responsive"
We've had a few hitches at Orchard Park Of Southfork. I've found myself peeking into their medication practices. But the new person in charge of activities has brought some positive changes. Sometimes the aides need to chill and remember they're dealing with folks who have memory issues like Alzheimer's. The place is super tidy and has nice decor. The staff here are kind and respectful. For the most part, we're really happy here because they really try to help you out.

"Keep Alert: Assisted Living Facility Fails to Meet Expectations"
Suggest you keep an eye on the types of activities available. We're really not happy with how they've been looking after her, especially with the activities in the memory care section. Laundry also has a habit of going missing and we end up with other people's stuff. The snacks they give out usually include cake or cookies. The staff could be more helpful and less rude. Thinking of putting in a nanny cam might not be a bad idea.

"Sorry about complaints. This was meant FOR A DIFFERENT HOME"
I lived at the assisted living facility for two months, from January to February. My main concern was with the Physical Therapy Contracted group. Some of them didn't seem to understand that most residents were elderly. They claimed to follow guidelines, but it felt like those guidelines were meant for people younger than 40, not an 82-year-old with knee injuries and a torn rotator cuff. The nursing care and aides were excellent except on weekends. Unfortunately, the food service didn't cater to the needs of a diabetic or someone on a mechanical soft diet.

"Friendly Staff, but Care and Safety Fall Short"
The facility is really clean and the staff are nice, it's just that the care we're supposed to get for our money isn't really there. We found a hearing aid under the bed once, and another time, a patient had broken a piece of glass and no one from the staff came to clean it up. The poor patient actually stepped on it.

Whenever I've visited, Orchard Park of Southfork buzzes with fun activities, and residents seem happy and involved. The staff is awesome. They always wear smiles and are super helpful, quick to lend a hand. The place shines with cleanliness, though occasionally I've caught a faint unpleasant smell. "Watch Out: Fancy Features Doesn't Mean Good Care"
This place sure has a lot of fancy features, but don't let that fool you. The care they provide isn't that great. If you're not able to be with your loved one all the time, and they can't really tell you what's going on when you're not there, I think you should definitely put a nanny cam in the room.
