"Extremely Poor Care at The Hamptons at Pineforest"
Our time at The Hamptons at Pineforest was downright awful. My family member only got bathed twice in three weeks. Even worse, they lied and penciled in extra baths on their records, but we knew what was up because we had a camera in her room. They even left her all by herself, even though she had a high risk of falling. She had to make two trips to the hospital in three weeks just for stitches. I wouldn't want anyone else to use this place.

"Friendly Staff and Good Food, Needs More Exercise Options"
I think this is one of the better places in Tyler. The staff is friendly and caring. The food is good and seems pretty nutritious, plus they give you plenty. Cleanliness is good but could be a bit better. Sometimes the floors don't look clean, but I know that's hard to keep up with. The activities they have, like bingo and other games, are fun. It would be nice if they had more of those. If a resident is having trouble, the workers will sit with them and help them join in, which is great. One thing I'd like to see more of is exercise. There's a lady who comes in the mornings to lead some easy exercises, but I think it would be good to have another session in the afternoon, especially since they don't have regular rehab and machines.

"Incompetent Management and Poor Care at Civitas Assisted Living"
Since Civitas took charge, the hygiene and attention in assisted living evaporated. The Executive Director lacks adequate skills to manage this place. They've seen too many changes in personnel and medication isn't handled properly. Expressing issues or worries falls on deaf ears. For my mom's sake, I shifted her to a different home.

"Management Fallout Diminishes Quality at the Hamptons."
After combing through prior reviews, I've decided to voice my own thoughts. The Hamptons has turned sour since the change of ownership in March of 2017. They brought a new executive director onboard around June or July of 2018, which was a terrible decision. The previous director was fantastic. He genuinely cared about the residents, their families, and his staff. He was hands-on, even willing to pick up the slack on his days off. But this new director? Terrible. She shows no compassion or consideration for the residents, the staff, or families. Instead of stepping in when needed, she lets the staff struggle. The personnel turnover has been immense since she and Civitas arrived. Employees who were there since the foundation was laid in 2008 have all gone. No effort was made to retain these valued workers. We once had a family member in the memory care unit here for a considerable stretch, but eventually relocated her due to the deteriorating conditions. The lone staffer versed in memory care and who showed genuine care for everyone is gone now. She continually exceeded all expectations and, to be frank, she could have operated the entire building with her leadership qualities. Even when she gave her notice, they didn't fight to keep her around. Unless Civitas identifies and addresses the true issue at hand, they'll continue to experience staff turnover and resident migrations. They would do well to look back at their past employees and understand just how valuable they were. It's tragic to see what was once a thriving company reduced to nothing.

"New Leadership Neglects Residents & Workers at The Hamptons"
I can firmly say that The Hamptons was once wonderful. Everything changed once Civitas Senior Living stepped in and brought it under their control, bringing with them a new executive director whose name I won't mention. This director pays no attention to the people living in the facility nor the hard-working employees. The leadership here is dreadful. All day she just stays in the office, showing no interest in having any sort of interaction with the team, the senior citizens living there, or their families. They are seriously understaffed, a problem that pertains not only to memory care but the assisted living sector too. Last Saturday is one such instance when there was no medical aid present in the memory care department for nearly five hours. This forces me to ask if the residents did receive their necessary medication? My worries are always brushed aside and passed on to the nurse.

"Understaffed, Inadequate Assistance and Medication Errors in Assisted Living"
My mom shifted to memory care from assisted living, about six weeks ago because she kept falling. But, the falls became even more frequent after she was moved. The memory care department is grossly under-staffed. The sole authority in charge is the executive director. When we transferred my mother, there were discrepancies between the provided medication list and the actual medications received. My mother suffered a reaction to a medication, leading to a situation where either I had to comfort her past midnight or she would have been sent to a mental health hospital. She is a dementia patient, not a mental health patient. I definitely would not suggest this facility to anyone.
