"Inconsistent care; lacks nurse. Okay for independent seniors, not others."

The assisted living has been like a rollercoaster. Some months were great, but then the staff changed, and it got scary. It's been inconsistent. The first six months were awesome, and I would've given it top marks. But everything changed in November when the director and staff changed. It started getting a bit better in the last month. Right now, it's okay for someone who doesn't need much help, like my mom. But if someone needs more care, it's not safe because they don't have a nurse anymore. They've been saying they're looking for one for four months now. My mom pays extra for transportation and medicine services, which adds up. Thankfully, she has home health to help her out.

"Caring Staff but Limited Food Variety and High Turnover"

I really liked the managers. They always had time for my questions and knew all the residents really well. The man who handled her medications was very organized. The only problem she had was that there wasn't enough variety in the food, and honestly, it didn't look good either. It was the same old thing every week. There was a lot of turnover among the aides, but the ones who stayed were kind and patient with her and us."

"Positive Changes at Assisted Living Facility!"

They recently hired a new Executive Director, and things are getting better. The staff who work there are incredibly caring.

"Staff Attentiveness with Minor Hiccups"

I’d suggest them. Overall, it felt good. The staff paid close attention, but some staffing issues affected meal service.

"Friendly Staff, Room Cleanliness Concerns"

I recommend this place. They’ve been great to work with. There are some issues with keeping things clean, though. My family member’s room isn’t cleaned every week, and it feels like we’re paying for that service. The staff is very friendly, and I believe my loved one is well cared for. The food could be better. We chose this facility due to its cost-effectiveness and quick availability.

"Assisted Living Tour Experience: Great, but Not Ideal Location"

We visited Truewood by Merrill in Cottonwood Heights and had a great tour experience. However, due to the location, it wouldn't be the right fit for our loved one and family.

"Great Care"

We chose this place because of near from A Place for Mom. It was tough deciding, but our family member felt well-cared for, and that's what counts most. I'd recommend it.

"More Activities Needed for Residents"

We chose this place because it’s close to home and the first one we visited. But they should offer more activities for residents.

"Peaceful experience with caring staff and beautiful surroundings"

We had a good experience at Truewood Cottonwood Heights. My mom was only there for a little over a month. She passed away peacefully at nearly 103 years old, though she liked to say she was already 103! The staff was very upbeat and kind. We especially appreciated the nurse/healthcare director and the aide. My mom chose this place partly because Cottonwood Creek runs through the property.

"Assisted Living Facility Offers Value Despite Food Concerns"

Everything has been pretty good overall. However, the food isn't always enjoyable, so I bring frozen dinners for my relative to ensure he has something he likes in his freezer. Still, considering the cost, you can't find just a room for rent in this area. Besides a room, he receives care, transportation, meals, and activities, making it a good deal. The staff has been very helpful, and the room is comfortable and well maintained.

" "Awful Experience at Canyon Creek, Better Options Available""

Our time at Canyon Creek was awful. We tried to make things work with them for 8 months before we finally moved our mom to a great Memory Care unit. Now, she gets compassionate and skilled care and we don't need to worry. While some of the nursing assistants at the facility tried their best, there simply weren't enough staff and they weren't properly trained to take care of those in memory care. The staff turnover rate was really high. Mom's move into memory care was really tough and terribly managed. We constantly had to make sure if mom took her medicines, took a shower, and wore her Depends. Sometimes she was cared for, but often times, she wasn't. She was mostly left to just lie in bed. Her room was dirty, the communal bathroom was revolting and not cleaned, even after we requested it. The last straw was when she fell and got badly bruised - and they didn’t even tell us about it. There are much better places out there. Keep searching!

"Horrible Stay at Canyon Creek, There Are Better Options"

Our experience at Canyon Creek was really awful. We tried to make it work for 8 months but in the end, we had to move our mom to a fantastic Memory Care place where she gets understanding, capable care and we can relax. A lot of the CNA's were doing their best at Canyon Creek but there weren't enough of them and they weren't properly trained to handle the challenges that come with memory care. Workers kept quitting all the time. The way they introduced our mom into memory care was really distressing and poorly managed. We always had to check whether they gave our mom her medicine on time, if they bathed her, or if she had on her Depends. Sometimes, they made sure to meet all her needs and many times, they ignored her needs. Our mom spent most of her time lying in bed. Her room wasn't clean, the shared bathroom was just gross and they wouldn't clean it even when we asked them to. What made us decide to move our mom was when she had a fall, got badly bruised and nobody let us know about it. If you are looking for a place for your loved one, there are way better options out there. Keep looking!

"Disappointment with staff, cold food and lost package"

My mom is in the assisted living area while my dad is in the unit for people with memory issues. Mom doesn't enjoy it too much here, complaining about the food in the memory care unit. She says it lacks variety and often arrives cold. I sent her a package once, properly labeled but it got lost. Also, there aren't any phones in the memory care unit, so contacting mom when she's visiting dad is a struggle. I also feel the facility needs more staff.

"Stay Away: Horrible Care and Mismanagement at Assisted Living Facility"

UNDER NO CONDITIONS SHOULD YOUR BELOVED BE STAYED HERE. This place was really awesome in the past; so loving caregivers and splendid services. But the service quality plunged since last autumn. They improved the appearance of the facility, which seems fantastic, but the meals are pretty bad, multiple medication errors, negligence, and more. They didn't provide any care to my dad, despite promising me a refund. Yet, I keep getting recurrent bills. The staff who were once fabulous, they all left, and they hardly manage to keep good personnel there. They are very short staffed and the new people have no idea how to do their job, plus they don't seem to care.

""Took a Look Around - Not for My Parents""

The place looked really old. Right when we arrived, they told us they were planning to renovate. I saw electric heaters in the windows of the rooms. The people who work there were friendly, but my parents wouldn't be comfortable here.

"Concerns about distance and assistance in assisted living facility."

This place was pretty good. The rooms were nice, but I felt like they were far from the dining area and activities, which I didn't like. I worried about how fast my loved one could get help because the place was big. It seemed like they'd need to be really independent to stay here.

"Friendly Staff, Nice Amenities"

We went to check out Canyon Creek and it was really nice. The place was super clean and tidy. The people who worked there were really nice and knew a lot. They answered all our questions, which was great. We liked all the cool stuff they had for us to use. We wanted a smaller place and Canyon Creek seemed just right because it felt cozy.

"Great Amenities, Wrong Location"

I visited this community recently for my loved one, and I liked what I saw. The place was clean, and they had lots of great amenities for all the residents. The activity list and meal plan were impressive. However, in the end, it wasn't the right location.

"Fantastic Care and Fun Activities Every Day!"

I’m really happy with how things are going for my loved one since she moved here. The staff are fantastic, and I feel good about the care she’s getting. The activities are awesome; there’s always something happening to keep everyone busy. I also like that the staff will bring her meals if she doesn’t want to go to the dining room. Overall, I’m very satisfied and would definitely recommend this place.

"Great care for my uncle at this community!"

I really loved this community. While my uncle lived here, he was doing really well. The staff was great at keeping an eye on his health. They even refused to give him soda when he asked, which helped him manage his sugar limits. I was really satisfied with everything they do.

Visited this facility and attended some of their activities. They have a full schedule of activities, which I was very impressed with. The facility is very clean and the staff seemed to go out of their way to help you. The food was delicious, tasted just like restaurant food. The Outreach director did a great job on our tour, and was able to answer all my questions. I would highly recommend Canyon Creek. A Big thumbs up. "Very Happy with This Community, My Sister is Thriving"

My sister is doing great here. The staff are really nice, and I’ve had no issues with them. They keep the place and her room clean and tidy. She’s starting to join some activities and likes the exercise class. The meals are good, but she thinks the portions are too big. I know she’s safe and well cared for. I would recommend this place.

"A Perfect Match"

We picked Canyon Creek because we believe it's the best place for Myrna. The community feels like a good fit. There are other residents similar to her, and the circular room layout allows her to move around easily. The Bridges program is also a plus.

"Be Aware: Mixed Experience at Assisted Living with Potential Concerns"

My mom was a resident for about half a year at this facility. The people working here are usually friendly and my mom, who's usually easy to get along with, stayed here in both the assisted living and memory care sections. However, if you want a place where your family-member would receive immediate care and close monitoring, I wouldn't recommend this place. Especially, if they're still moving around on their own, because there can be risk of frequent falls. Another downside is they never told us to label her clothes. There's no lost and found for unlabeled or unclaimed clothes, so any unmarked clothes just get given to someone else. Plus, they serve alcohol in an activity they call "Margarita Madness." I don't think it's appropriate for people with dementia who might be on different meds that could negatively interact with alcohol.'

"Needs Improvement in Welcoming New Residents"

I believe the low scores are because they could improve how they welcome new residents. They should have more activities that help everyone get to know each other better. Also, they need to plan activities that more residents would enjoy.

It's clean, and the building layout is easy for seniors to get around. I'm always impressed by how much the staff seem to enjoy working there, and they all pitch in to make sure residents are well cared for. I want to take a moment to thank you for the amazing care you give to my father. Before he came to your community at the end of August, he was not doing well. He seemed to have lost his spark and his will to live. But since moving here, he has really come back to life! He is happy now, and his health is getting better. Your staff has taken such great care of him, and I couldn't be happier with our decision to move him here. I've seen many facilities around the Salt Lake Valley, and I truly believe that the care and service your staff offers is the best. My mother was supposed to stay at Canyon Creek, but she changed her mind two days before moving in. Although my review is based on limited experience, I wasn't thrilled because when we signed up, we paid some fees including an evaluation fee. I knew some of the money wouldn't be refunded, but I felt we should've got some back since she never actually stayed there. Despite all this, I really liked Canyon Creek. "Review of Small and Costly Assisted Living Facility for Mother"

Not a lot of space to roam. The care and staff are okay. It's located in a busy retail/dining area. The cost is $3600 per month, but there are also a lot of extra charges. You need to pay another month's rent as an administrative fee. In my view, it's not suitable for my mother.

"Mixed Experience at Canyon Creek Assisted Living Facility"

My mom moved to Canyon Creek but sadly passed away just a month later. Because of the short time spent there, we couldn't figure out her therapy or medications properly. Once she moved in, she was more or less left to navigate on her own. I tried to assist but didn't know who to approach with my concerns. Initially, someone tried to understand her hobbies but after that, she was mostly on her own. I hoped they would have helped her mix with the community better. The activities and food are decent but not exceptional. At meals, seats are determined but her table mates often ate at different times, so she was alone most of the time. One key reason mom chose Canyon Creek was to have her dogs with her. We paid extra so that the nursing staff would walk them once every shift, but this didn't happen consistently. Mom wasn't feeling well to raise this issue and the staff didn't check either. Since her stay was short, we could not address this problem. On a positive note, the nurses, CNAs, and dining room staff were very caring and considerate. Given more time, my opinion about the facility might have improved. Also, we had to miss our planned "care conference" due to my mom's health. I believe most of our problems could have been resolved had we managed to meet everyone in a single meeting.

"Tour guide avoids showing 'real' care area – unsettling"

This place just didn't win me over. They claimed we needed special care for memory. They took me for a tour and had me in this pretty amazing model room, then treated us to a real nice lunch. But that wasn't gonna be our actual spot. I had to see the area for memory care and where folks would be eating. The tour guide wasn't keen on showing me that. It didn't add up - I'd be seeing that area eventually if we stayed there. If they're hiding it before we join, that's sort of sketchy. It didn't exactly get me trusting their honesty or openness.

My mom says she isn’t getting all the services she’s supposed to. I need to look into that because she might not know exactly what she should be receiving. They have great activities and are very friendly. It would be nice if they made it a bit easier to reach someone. We had some trouble getting in touch with people sometimes.

"Cozy, but Dark: Assisted Living with Furry Friends"

The building setup wasn't great. It felt gloomy and cramped. Still, nice views overall. Loved their resident dog, a charming touch. Even a nurse brought her dog. Parents enjoyed it.

"Friendly Staff and Plenty of Activities at Canyon Creek Assisted Living"

Canyon Creek's staff are awesome! Compared to other places, they're really nice. They have a doctor and CNAs there, too. The place looks great, and the cost is similar to others. They've got lots of activities, from trips to games.

"Not bad, but noisy location near Fort Union."

Pretty nice, but I'm not a fan of the location right by Fort Union—it's noisy.

"Clean but pricey assisted living with no pets allowed."

This facility feels warm and inviting, it is always tidy and well maintained. It's a great place for residents to enjoy the outdoors when the weather is pleasant. However, it's more costly than predicted and doesn't allow pets which might be a bummer for some. Also, it's situated pretty far to the east side. Currently, there are 5 people waiting to get in.
