"Poor conditions and lockdown upon arrival; not recommended."
The place isn't recommended. When I moved my mom in, they locked down. Six weeks later, when I shifted her to memory care, was the first time I saw her room. The bathroom was shocking (I have pictures): poop in the toilet, stains in the sink, and dirt by the baseboard. It didn't happen in a week. The staff were nice, but I can't suggest them to anyone.

"Good Community, but Expensive Medications and Average Communication"
The only problem I had was with the medication. Their pharmacy services cost more than they should. Communication between the staff and the family was okay, we got updates but not as often as we would like. I would recommend this community to others.

"Facility Declined Over Time"
We picked this place based on what A Place for Mom suggested. My mom's bathroom was dirty a lot. She needed memory care more than just regular assistance, but they didn't want to help. There weren't any staff around, and I had to stay three nights because she wandered. At first, I liked this place, but it got worse toward the end.

"Friendly Staff, Room Cleanliness Lacking"
We hoped for more frequent cleaning of my mom’s room. The people working there were super nice, and the person in charge was really kind too. We think Fox Trail Assisted Living At Stephens is worth the money.

"Appalling Conditions Discovered During Unexpected Visit to Facility"
This place is unclean and revolting. I took a tour intending to find a home for a loved one and they weren't even allowing me to inspect every vacant room. A day shift worker let slip that there were other rooms ready for rent in a separate hallway. The overwhelming scent of urine combined with a vinegar-like odor was nauseating. I made a surprise return visit and had the same worker, whose name I've chosen not to disclose, guide me around. Let's just say, if they aren't expecting visitors, cleanliness is not a priority. Residents were practically tumbling out of their chairs and there was a foul feces smell lingering. Honestly, it's nothing short of disgusting.

"Cleanliness and Space Concerns in Assisted Living Facility"
Was really nice and told us lots of helpful stuff. But this place wouldn’t suit our mom. It didn’t seem very clean, and the rooms were tiny and had to be shared. We'd rather find a place with a studio or one-bedroom option.

"Loss of Excellent Chef Due to Staff Bullying"
I just discovered that the top chef in this facility quit because she was being treated poorly and picked on by other returning staff. This is a great loss for the residents here. Her meals were fantastic and fresh, and it was clear she took into account what the residents were eating. So disappointing! This place needs to be looked into.

"Mixed Feelings with Good Care: Assisted Living Review"
My husband's adjustment to this community is ongoing. Some days are tough, but the staff does a good job caring for him. Though it's not like home, I feel he's safe and well-cared for here. The community recently changed ownership, so there are some growing pains. The new activities director is excellent, and the meals are good. Overall, he receives good care here, and I would recommend it.

"Adjusting to Change, Yet Overall a Nice Community"
My husband is still getting used to his new home here. Some days are tough, some are good. But I'm happy with how the staff is looking after him and his needs. It's not like being at home, but I'm confident he's safe and well cared for. There have been some bumps since new ownership took over, but there's a great new activities director. The meals are good, and I feel he's in good hands. I'd recommend this place.

This place has new management and they're busy remodeling. The staff takes care of my mother-in-law, but there aren't enough activities for residents. They could improve cleanliness too.

My mom's place at the assisted living facility has been mostly good. The only real problem is that the WiFi doesn't reach her room. They're working on fixing it, though. The people there have all been nice and helpful to her, and she says the food is pretty good.

My mom really enjoys this place. After moving from one assisted living facility to another, this is the first place she’s truly happy with. They really make you feel like family. Everything is wonderful here. I recommend Amerisist to anyone who wants the best for their loved ones. This is the place to be."

"Assisted Living Facility Falls Short in Care for Residents"
The first few weeks were okay. My mother-in-law liked it there. But I noticed something concerning. Some residents who couldn't move in wheelchairs were just left in the hallway or dining room. Some looked uncomfortable. I was relieved my mom could move somewhat. She'd been there for a month when my husband found her soaked in urine on a Saturday morning. Her speech was also slurred. He called an ambulance. Sadly, she'd had a massive stroke unnoticed by the staff. She passed away a week later. I don't think they have enough or well-trained staff to care for non-mobile or verbally impaired residents. I wrote to the CEO expressing our disappointment and asked for a refund two weeks ago. No response yet.

"Avoid Amerisist for Quality Elderly Care"
The first couple weeks seemed okay and my mother-in-law seemed to enjoy her time at Amerisist. But pretty soon, I noticed something disturbing. Some residents who couldn't move themselves in their wheelchairs were just left neglected in hallways or the eating area, some even falling over. Luckily, my mom could move a bit on her own. After about a month, my husband went to visit her on a Saturday around 11:00 and found her in an unacceptable state. She was soaked in urine, her depends and clothing all wet. He noticed she was talking funny, too. So, he quickly had them call an ambulance. Turns out, she had a massive stroke and no one there had even noticed. Sadly, she passed away just a week later. I don't think Amerisist has enough workers, especially ones that are properly trained to look after residents who can't move around much or talk properly. I wrote a letter to their CEO about two weeks ago expressing our disappointment and asking for a refund but, to this day, got no response.
