"Unsafe and Unreliable: Oak Park Place Review"
My family member went to Oak Park Place for rehab. It was awful. Once, she ended up in the hospital from taking too much medicine. Twice, she got someone else's meds. The rehab part was okay, but the staff were always too busy. Sometimes, we couldn't even find them. The social worker and boss never want to talk face-to-face about problems. Once, they hid in their office when we tried to talk. That's a big red flag! I wouldn't go back or tell anyone to go there.

"Good care but needs better communication with family"
I can imagine that working in a memory care unit is really tough for the staff. They need to be like mind readers and have a lot of patience when helping people with dementia who might be angry, violent, or not able to speak. Because of this, I think the staff should talk to family members more often. My main issue is the lack of communication. Sometimes I see staff I don’t recognize, and I’m not sure who is in charge. But overall, I believe they do their best.

My loved one lived at Oak Park Place Madison, and I visited once a week. Overall, he had a good experience there. The staff seemed nice, though they were quite busy. Some staff were better than others. The place appeared clean. Communication with the family could be improved, as I didn't have a specific contact person. However, hospice did keep me informed. Families should make sure they have all necessary contact details.

We visited Oak Park Place Madison while looking for assisted living for our family member. The place seemed pretty good. It was very clean and looked new. The staff were friendly and helpful.

"Assisted living facility lacks adequate staffing and care for residents."
My family member has been at the Oak Park Place location on Nakoma Road for about a month. We visited twice and thought it was nice. However, there were very few people around during our visits, which worried us. Despite that, my loved one liked the apartment. We figured it would be okay. The rooms are nice, and a few staff members are good at their jobs. But overall, they don't have enough staff in many areas, and the ones on duty seem inexperienced. They struggle to provide the care my loved one needs. Considering the cost, it's very disappointing. I understand it's hard to hire and keep staff at care facilities, and residents can be challenging. Still, I wouldn't recommend Oak Park Place Nakoma.

I had a really good tour at Pak Park Place Madison. The place was clean and seemed to be well managed. They offer independent living, assisted living, and memory care.

"Cleanliness Needs Improvement"
I think they should make sure the rooms for the residents are kept clean. The room my family member stayed in was really dirty. I asked for a vacuum to clean it myself a lot of times, but they said they didn't have one. The bathroom hardly ever got cleaned, so I had to do it myself. Because of this, my family member doesn't live there anymore.

Oak Park Place seemed nice, but it was too big. We worried our loved one might get lost. It was new during our tour, and not many people were there, so it was hard to know what it'd be like.

"Nice Facility, but Activities Spread Out Concern Us"
Our family visited Oak Park and it’s a really nice place. Usually, they let families eat with the residents during the tour, but because of COVID, they gave us a meal to take home instead. We thought the meal was quite good. Our main concern is that the activities are spread out on different floors. If our loved one started having memory problems, we wouldn’t want them to get lost.

"Great activities and staff, but needs more care options"
Our stay was brief, but we liked Oak Park Place. We chose it because my mom had a friend who had a good experience here. They offer all levels of care, which is nice. My mom had a studio that was a good size with a big bathroom for wheelchairs. I really liked the many activities, the good food, and how friendly and accommodating the staff were. I would recommend this place. However, I think they could offer a bit more care beyond just memory care. It's also great that they have rehab right across the street.
