"Unsatisfactory Care and Poor Food Choices"
The staff here aren’t doing their work properly. They don’t complete tasks they’re supposed to. Ever since my family member came here, we’ve had problems with the care they’re getting. My relative really dislikes the meals here. The menu has hot dogs and other foods that seem more suitable for kids. They’ve only just begun offering activities. There’s no difference between the memory care and regular assisted living here. I wouldn’t suggest this place to others.

"Staffing Issues, Great Director, Good Cook"
There have been lots of issues finding and keeping staff, like everywhere else. They do their best, but it's been tough. They can't find an activities director, so there isn't much going on. The cook they have now is really good. The director is amazing. She communicates well with the family and handles issues as best as she can.

"Headline: No Hot Water in Assisted Living Facility"
During my dad's first month here, there was no hot water anywhere. This place is run by Fleming Corporation. Despite the staff's multiple requests, nothing has been done about this issue. Now, they want my parents to provide 30-day notice, without even guaranteeing that hot water will be available. Regrettably, I can't even give them a single star. I'm left wondering how they're managing to clean the dishes?!

The assisted living facility felt a bit chaotic when you first walk in. Everything and the gathering areas were right there, which made it seem louder. However, the rooms were quieter, and some common areas were calm. Overall, we liked the facility, but it didn't quite feel like home compared to others we visited.

The place seemed okay, rooms were big, lots of storage, nice stuff. It’s peaceful here. But I wish the doors weren’t always locked. I get it, but I want my mom to be able to go outside whenever she wants.

Since my mom moved here, things have improved. The old managers didn't keep their promises. But the new ones are kind and helpful. My mom feels safe with the pull cord for help. I'm not sure if I'd suggest this place, but it's getting better.

"Caring staff and clean environment for my wife"
My wife is being taken care of really well here. I'm mostly comfortable with her move to this community. It's a nice place, and everything is kept up nicely. They do a great job with cleanliness. I really like the staff; they've been helpful and easy to work with. I feel confident she's safe and in good hands. I don't see why I wouldn't recommend this place—it's nice.

My wife is being cared for very well at this community. I’m mostly comfortable with her move here. It is a very nice place, and everything is well maintained. They do a great job keeping it clean. I really like the staff—they’ve been very helpful and great to work with. I know she is safe and in good hands. I see no reason not to recommend this community; it’s a nice place.

The folks at Sylvan Crossings in Westshire Village are truly great. They're super helpful and nice. Our family member has been doing okay since moving there. The place is kept really clean and neat. The food they serve is alright, nothing too fancy. But it's a long way from where our family lives and from the city. Also, they don't provide rides to the doctor, which makes things really tough for us.

The people who work at Sylvan Crossing in Westshire Village are really nice. They help us a lot and always have a friendly attitude. Our family member is doing okay since moving there. The place is clean and well taken care of. The food is okay, nothing amazing. But, it's really far from where our family lives and from the city. "Didn't have a positive experience at the care facility"
They barely ever paid a visit to check on him. The meals were awful. When I inquired if they provided help with day-to-day living, they said yes. But once I stayed there for a while, I noticed the residents needed more help than I did. There was almost no mingling and they never discussed my care plan. I wouldn't suggest this place to anyone.

"Loud Alarms and Mediocre Food Mar Decent Care"
My mom, who has Dementia, was put into this care facility for a brief period. All the staff was really nice and willing to help. They were caring and supportive towards my family and my mom during this tough time. But, the alarm noises in this place are super loud and annoying. I hope they're going to find a different way to give alerts. They should also work on improving the food. Whenever we had questions or worries, they seemed to handle them in a pretty timely way. I would suggest this place.

My mom likes the caregivers, they're friendly and helpful. They make her feel welcome. The place is clean and comfy. Staff listen to families, which is good. Food needs improvement, more healthy options, fewer processed meats. Exercise program could be better, especially for upper body strength. More variety in activities would be nice. The constant buzzing from call buttons is annoying. Thanks to staff for helping during the transition. Understanding problems fully is important.

Starting with the good stuff, my mom really likes the CNA staff. They listen to her, help when she needs it, and make her feel welcome. The CNAs create a pleasant atmosphere at Sylvan Crossings. The place itself is okay. It's clean and has nice furniture. The management team tries to listen to families during tough times, which we appreciate. But there are some areas that need improvement. First, the food quality isn't great. Hot dogs and bologna are made from processed meat, which isn't healthy. We need more veggies, fruits, and whole grains. And maybe switch to brown rice instead of white rice. The exercise program isn't exciting for my mom. They should offer more exercises and consider residents' physical abilities. Upper body strength is a challenge for her. Also, there's not enough variety in activities. Sylvan Crossings should ask residents what they want. Lastly, that buzzing noise from the call buttons is annoying. I hope they find a better solution. Despite the ups and downs, I appreciate the staff's efforts during my mom's transition. It's important to gather all the facts and solve problems.

Sylvan Crossing, situated in Westshire Village but labeled as Westport, initially confused my mother, a long-time local. Despite this, it's a pleasant place with outdoor spaces for walks and socializing. Smoking is permitted outside, and there are communal areas for watching TV. The staff is friendly and genuinely cares for the residents. However, the sign-in process needs improvement, with separate notebooks for visitors and residents. Mail handling also needs work; residents should have their own secure mailboxes. Overall, it's satisfactory. My mother receives the care she needs and is content.

Sylvan in Waunakee was the best of the three places in Dane County that cared for our mom, who had dementia. We had to leave the first place because of rising costs, and the second place had young, overworked staff who didn't seem guided. Sylvan is small, personal, and well-managed. It was a bit cheaper than the others but offered the services Mom really needed. The staff genuinely cared about her, helping her take showers or encouraging her to socialize at mealtimes. Mom met and interacted with other residents for the first time in Sylvan. She started to do better, which surprised us. When her health worsened, we got timely weekend and after-hours updates from Sylvan, plus hands-on care from staff who already knew her, even when we thought we might need to move her to hospice. Sylvan isn't big or fancy, but it was the perfect place for our family.

Sylvan in Waunakee was the best of the three places in Dane County that took care of our Mom, who had dementia. Rising prices made us leave the first place, and the second had young, overworked staff. Sylvan is small, personal, and well-run. It was a bit cheaper than the others, but it gave Mom the services she really needed. The staff truly cared about Mom. They took the time to help her shower when she didn’t want to and encouraged her to have lunch in the dining room to socialize. Mom met and talked with other residents in the living room, which was new for her. She started to thrive again at Sylvan, which surprised us. When Mom’s health got worse, we got timely phone updates from the staff, even on weekends and after hours. They also provided hands-on care in her room from people who already knew her, which made us feel better when we thought we might have to move her to hospice. Sylvan isn’t big or fancy, but it was perfect for our family.

"We’re incredibly happy with the care and friendliness here."
We’re really happy. The staff is super enthusiastic and eager to help the residents. I never thought a nursing care facility could be like this. It’s not as sterile as I expected. It’s very homey. Jack is very happy. They’re extremely friendly; I’d rate them a 6 out of 5. I was surprised at how reasonable the cost is. I don’t know much about the activities. Jack doesn’t use the recreational facilities because he’s too sick and introverted. He doesn’t join in games and such. They’ve been very responsive whenever I’ve had a question or concern. They respond immediately. The location is just down the road from us, and we’re there every day. I would recommend this to anyone looking for a safe and happy place for care beyond assisted living.

Toured with Allan. Had questions about TV and rules. Went again, no answers. Friend saw bad changes. Place looks messy. Only 3 rooms left. Allan called back late.
