"Improvements Needed at Assisted Living Facility"
We chose this community because it was the only place that would accept my loved one. The staff was friendly and caring. However, they should improve their activities since they lost their director. Consistency in care plan pricing is needed, and fresher fruits and vegetables would be a nice addition. The overall environment can be a bit noisy.

"Sylvan Crossings Provides Poor Care and Neglects Residents"
Our experience with Sylvan Crossings was very poor. A family member in our care got bed sores while living there and also contracted the coronavirus. The staff did not monitor her as they should have and some of her belongings even disappeared. She was left unattended the whole day in her room, fully dressed in her chair, without assistance to get to her bed or have her clothes changed. This means she ended up sleeping in the same clothes she wore during the day. Her hair was consistently messy, showing that they didn't bathe her regularly. Moreover, there was only one staff member on duty at night and she would often fall asleep. I cannot, in good conscience, recommend this facility to anyone.

"Decent Care, Some Room Issues, and Occasionally Hard to Reach Director"
Overall, the staff was usually attentive. They took good care of the residents, although there was one problem with my father's food at first. The common rooms were really nice. My father had a bit of trouble with his first room because the window wouldn't close all the way during winter. His second room was okay. Sometimes, it was hard to reach the facility director, but they did follow through eventually.

"Poorly Staffed Assisted Living with Mediocre Activities"
Go somewhere else. The staff was too small, and they made caregivers and nurses cook in the kitchen. They worked too much. The place wasn't clean, and they didn't take good care of the residents. All the food was frozen. They had a few activities, but they weren't very interesting. My loved one didn't like anything there. The staff changed a lot, which caused problems with medications.

I toured this community for my dad’s needs. The staff was friendly and informative, answering my questions. The place looked nice and well-kept. Overall, a positive experience. Although I found a better-suited community, my tour here was good.

At Sylvan Crossings, they personalize care for each resident. They understand that when loved ones with dementia show behaviors, they're trying to communicate. They assess situations, make necessary environment changes for comfort. I'll refer more patients here!

"Small, Liked but Distant; Recommended Nearby Westshire Village"
When we visited this community, we liked it, but it felt small with only about 32 residents. It's quite far from my home, so commuting would be a hassle. They suggested I check out Westshire Village instead, and I really enjoyed that one.

"Mixed Experience at Sylvan Assisted Living, Room Layout Not Ideal"
We visited Sylvan in Waunakee and met with [Removed]. She was friendly and helpful. The building, grounds, and common areas were well-maintained and pleasant. The resident setup included rooms with bathrooms and spacious common areas, along with cozy sitting areas throughout. Overall nice, but not suitable for an apartment-style setting that our Father needs.
